摘 要:目的:基于定量CT(QCT)探讨不同性别腹部脂肪与代谢综合征(MS)的相关性。方法:纳入2022年3月~2023年5月于弋矶山医院行骨密度QCT检查的1612例受试者,分为MS组(男314例,女122例)与非MS组(男704例,女472例),采用QCT获取两组的L3中间层面脂肪参数,包括总脂肪面积(TFA)、内脏脂肪面积(VFA)、皮下脂肪面积(SFA)及皮下/内脏脂肪面积比值(SVR),分别反映腹部总脂肪量、内脏脂肪量、皮下脂肪量及脂肪分布。采用多因素Logistic回归分析确定脂肪参数中MS的独立影响因素并绘制受试者工作特征(ROC)曲线。结果:男性TFA、VFA均高于女性(P<0.001),而SFA、SVR则低于女性(P<0.001)。男、女各组别中,MS组TFA、VFA、SFA均高于非MS组,SVR低于非MS组(P<0.01)。男性中,TFA、VFA升高是MS的独立危险因素(P<0.05),SVR升高是MS的独立保护因素(P<0.05),VFA诊断MS的曲线下面积(AUC)最高(0.851);女性中,TFA、VFA及SFA升高是MS的独立危险因素(P<0.05),TFA诊断MS的AUC(0.850)最高。结论:腹部脂肪与MS的关系存在一定性别差异,男、女腹部总脂肪及内脏脂肪、女性腹部皮下脂肪是MS的独立危险因素,男性腹部SVR是MS的独立保护因素;QCT可用于MS患者的筛查。Objective:To investigate the correlation between abdominal fat and metabolic syndrome(MS)in different genders based on Quantitative CT(QCT).Methods:In total,1612 subjects undergoing bone density examination by QCT in our hospital were recruited from March 2022 to May 2023,and divided into MS group(314 males and 122 females)and non-MS group(704 males and 472 females).QCT was used to obtain fat parameters at lumbar-3 vertebral level in two groups,including total fat area(TFA),visceral fat area(VFA),subcutaneous fat area(SFA),and subcutaneous/visceral fat area ratio(SVR),representing total abdominal fat,visceral fat,subcutaneous fat and fat distribution respectively.Multivariate logistic regression model was employed to determine the independent factors affecting the fat parameters in MS,and receiver operating characteristic(ROC)curve was generated.Results:The TFA and VFA in males were higher than those in females(P<0.001),while the SFA and SVR in males were lower(P<0.001).The TFA,VFA and SFA of both genders in the MS group were higher than those in the non-MS group,whereas the SVR was lower than that in the non-MS group(P<0.01).Among males,elevated TFA and VFA were independent risk factors for MS(P<0.05),elevated SVR was an independent protective factor for MS(P<0.05),and the AUC(0.851)of VFA for diagnosing MS was the highest.Among females,elevated TFA,VFA and SFA were independent risk factors for MS(P<0.05),and the AUC(0.850)of VFA for diagnosing MS was the highest.Conclusion:There are certain gender differences in the relationship between abdominal fat and MS.Total abdominal fat and visceral fat in both genders and subcutaneous fat in females are independent risk factors for MS.SVR in males is an independent protective factor for MS.QCT can be used for screening MS patients.
