心速宁胶囊联合胺碘酮治疗室性早搏的临床研究

摘　　要：目的 观察心速宁胶囊联合胺碘酮治疗室性早搏的疗效。方法 依据随机数字表法将124例室性早搏患者分为对照组(62例,胺碘酮)与研究组(62例,心速宁胶囊联合胺碘酮)。比较临床疗效、中医证候积分、动态心电图结果、心功能、一氧化氮、内皮素-1、纤溶因子及药物相关不良反应。结果研究组的总有效率高于对照组(P<0.05)。治疗后,2组的中医证候积分、室性早搏次数、ST段下移时间、ST段压低距离、内皮素-1、纤溶酶原活化剂抑制物-1(PAI-1)低于治疗前(P<0.05),且研究组低于对照组(P<0.05)。治疗后,2组的每搏输出量(SV)、左心室射血分数(LVEF)、一氧化氮、组织性纤溶酶原激活物(t-PA)高于治疗前(P<0.05),且研究组高于对照组(P<0.05)。研究组的药物相关不良反应总发生率较对照组差异无统计学意义(P>0.05)。结论 心速宁胶囊联合胺碘酮可改善室性早搏患者的动态心电图结果、心功能,降低中医证候积分,调节一氧化氮、内皮素-1、纤溶因子,增加临床疗效,且未增加药物相关不良反应。Objective To observe the therapeutic effect of Xinsuning capsules combined with amiodarone in treating premature ventricular contractions.Methods According to the random number table method,124 patients with premature ventricular contractions were separated into a control group(62 cases with amiodarone)and a study group(62 cases with Xinsuning capsules combined with amiodarone).Clinical efficacy,traditional Chinese medicine syndrome score,dynamic electrocardiogram results,heart function,nitric oxide,endothelin-1,fibrinolytic factor,and drug-related adverse reactions were compared.Results The total effective rate of the study group was higher than that of the control group(P<0.05).After treatment,the traditional Chinese medicine syndrome score,number of premature ventricular contractions,ST segment descent time,ST segment depression distance,endothelin-1,plasminogen activator inhibitor-1(PAI-1)in both groups were lower than before treatment(P<0.05),and these indexes in the study group were lower than those in the control group(P<0.05).After treatment,the stroke volume(SV),left ventricular ejection fraction(LVEF),nitric oxide,and tissue plasminogen activator(t-PA)of the two groups were higher than before treatment(P<0.05),and these indexes in the study group were higher than those in the control group(P<0.05).Drug-related adverse reactions showed no statistically significant difference between the two groups(P>0.05).Conclusion The combination of Xinsuning capsules and amiodarone can improve the dynamic electrocardiogram results and heart function of patients with premature ventricular contractions,reduce traditional Chinese medicine syndrome scores,regulate nitric oxide,endothelin-1,fibrinolytic factors,increase clinical efficacy,and do not increase drug-related adverse reactions.
