超声引导下臂丛神经阻滞麻醉与静脉全麻在肩周炎手法松解术中的应用效果比较

摘　　要：目的比较超声引导下臂丛神经阻滞麻醉与静脉全麻在肩周炎手法松解术中的应用效果。方法选取2021年1月至2022年6月收治的80例肩周炎手法松解术患者为研究对象,根据麻醉方式不同将其分为对照组和观察组,各40例。对照组采用静脉全麻,观察组采用超声引导下臂丛神经阻滞麻醉。比较两组的麻醉效果。结果切皮时(T_(1))~术毕(T_(4)),观察组的心率(HR)、平均动脉压(MAP)均低于对照组(P<0.05)。术前(T_(0))~T_(4),两组的超氧化物歧化酶(SOD)、肾上腺素(E)水平无明显差异(P>0.05)。观察组的麻醉阻滞起效时间短于对照组,术后2 h视觉模拟评分法(VAS)评分低于对照组(P<0.05)。观察组的不良反应总发生率低于对照组(P<0.05)。结论超声引导下臂丛神经阻滞麻醉可提高肩周炎手法松解术患者的麻醉效果,改善血流动力学指标,缩短麻醉阻滞起效时间,也能减轻术后疼痛,提高安全性,值得推广。Objective To compare the application effects of ultrasound-guided brachial plexus block anesthesia and intravenous general anesthesia in manual release of shoulder arthritis.Methods A total of 80 patients undergoing manual release of shoulder arthritis from January 2021 to June 2022 were selected as the study objects and divided into control group and observation group according to different anesthesia methods,with 40 cases in each group.The control group was adopted intravenous general anesthesia,and the observation group was adopted ultrasound-guided brachial plexus block anesthesia.The anesthetic effects of the two groups were compared.Results At skin incision(T_(1))-after operation(T_(4)),the heart rate(HR)and mean artery pressure(MAP)in the observation group were lower than those in the control group(P<0.05).Before operation(T_(0))-T_(4),there were no significant differences in the levels of superoxide dismutase(SOD)and epinephrine(E)between the two groups(P>0.05).The onset time of anesthesia block in the observation group was shorter than that in the control group,and the Visual Analogue Scale(VAS)score at 2 h after surgery was lower than that in the control group(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion Ultrasound-guided brachial plexus block anesthesia can enhance the anesthetic effect in patients undergoing manual release of shoulder arthritis,improve hemodynamic indexes,shorten the onset time of anesthesia block,reduce postoperative pain and improve safety,which is worth popularizing.
