摘　　要：目的探究四分之一定位及触摸法联合便利蜷曲对重症肺炎新生儿桡动脉采血疼痛程度及生理应激反应的影响。方法选择2019年9月至2021年10月收治的130例重症肺炎新生儿作为研究对象,根据入院时间将其分为对照组(2019年9月至2020年9月)和观察组(2020年10月至2021年10月),每组65例。对照组接受常规护理,观察组在对照组干预基础上给予四分之一定位及触摸法联合便利蜷曲进行干预。比较两组的桡动脉采血疼痛程度、生理应激反应指标、一次穿刺成功率及家属护理满意度。结果穿刺时及穿刺后1、10 min,观察组的新生儿疼痛评估量表(NIPS)评分均低于对照组(P<0.05)。穿刺时、穿刺后30 min,观察组的心率、呼吸频率均低于对照组,血氧饱和度高于对照组(P<0.05)。观察组的一次穿刺成功率、家属护理满意度均高于对照组(P<0.05)。结论四分之一定位及触摸法联合便利蜷曲用于重症肺炎新生儿桡动脉采血护理中,不仅能减轻新生儿桡动脉采血疼痛及生理应激反应,还能提高一次穿刺成功率及家属护理满意度,值得推广。Objective To explore the effect of quarter positioning and touch method combined with convenient curling on pain degree and physiological stress response of radial artery blood collection in neonates with severe pneumonia.Methods A total of 130 neonates with severe pneumonia admitted from September 2019 to October 2021 were selected as the research objects,and the neonates were divided into control group(September 2019 to September 2020)and observation group(October 2020 to October 2021)according to the time of admission,with 65 cases in each group.The control group received routine nursing,and the observation group was given quarter positioning and touch method combined with convenient curling intervention on the basis of the control group intervention.The pain degree of radial artery blood collection,physiological stress response indexes,one-time puncture success rate and family nursing satisfaction were compared between the two groups.Results At the time of puncture,at 1 min and 10 min after puncture,the score of Neonatal Infant Pain Scale(NIPS)in the observation group was lower than that in the control group(P<0.05).At the time of puncture and at 30 min after puncture,the heart rate and respiratory rate of the observation group were lower than those of the control group,and the blood oxygen saturation was higher than that of the control group(P<0.05).The one-time puncture success rate and family nursing satisfaction of the observation group were higher than those of the control group(P<0.05).Conclusion The quarter positioning and touching method combined with convenient curling in the nursing of radial artery blood collection in neonates with severe pneumonia can not only reduce the pain and physiological stress response of neonatal radial artery blood collection,but also improve the one-time puncture success rate and the family nursing satisfaction,which is worthy of promotion.
