摘 要:目的探究经颅直流电刺激联合依达拉奉治疗急性重度一氧化碳中毒的效果。方法选择联勤保障部队第九〇四医院急诊科在2019年6月—2023年6月收治的82例急性重度一氧化碳重度患者为研究对象,按随机表分为两组,各41例,两组均进行氧疗、营养脑细胞等支持性治疗,在此基础上对照组进行依达拉奉治疗,观察组进行经颅直流电刺激联合依达拉奉治疗。观察两组的临床疗效、意识状态(GCS评分)、认知功能(MMSE评分)、神经损伤标志物水平、迟发性脑病与不良反应发生率。结果观察组的总有效率(92.68%)高于对照组(75.61%)(P<0.05);治疗后,观察组的GCS、MMSE评分高于对照组,神经元特异性烯醇化酶(NSE)、S100钙结合蛋白(S100B)水平低于对照组(P<0.05);观察组的迟发性脑病发生率(9.76%)低于对照组(29.27%)(P<0.05);比较不良反应发生率,差异无统计学意义(P>0.05)。结论经颅直流电刺激联合依达拉奉治疗急性重度一氧化碳中毒患者具有良好疗效,改善患者意识状态与认知功能,减少神经损伤与迟发性脑病的发生。Objective To study the effect of transcranial direct current stimulation(tDCS)combined with Edaravone in the treatment of patients with acute severe carbon monoxide poisoning(ASCMP).Methods A total of 82 cases of ASCMP who were admitted to the Emergency Department of 904th Hospital of the Chinese People's Liberation Army Joint Logistics Support Force from June 2019 to June 2023 conducted on this study,All patients were randomly divided into two groups.Both groups received supportive treatments such as oxygen therapy and nutrition of brain cells.The patients(41cases)of the control groupreceived Edaravone treatment on this basis.The patients(41cases)of the observation group received tDCScombined with Edaravone.The clinical efficacy,status of consciousness(GCS score),cognitive function(MMSE score),markers of nerve injury,incidence of delayed encephalopathy and adverse reactions were observed in both groups.Results The effective rate of the observation group(92.68%)was higher than that of the control group(75.61%)(P<0.05).After treatment,the scores of GCS and MMSE in the observation group were higher than those in the control group,and the levels of neuron-specific enolase(NSE)and S100 calc-binding protein(S100B)in the observation group were lower than those in the control group(P<0.05).The incidence of delayed encephalopathy in the observation group(9.76%)was lower than that in the control group(29.27%)(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion tDCS combined with Edaravone has good efficacy in treating patients with ASCMP,improving the patient's state of consciousness and cognitive function,and reducing the occurrence of nerve damage and delayed encephalopathy in this study.
