黄油膏外涂预防重症患者腹泻所致肛周皮肤损伤的效果评价

摘　　要：目的评价黄油膏外涂预防重症患者腹泻所致肛周皮肤损伤的效果。方法选取2021年3月至2023年3月茂名市中医医院重症监护室收治的80例重症患者(伴腹泻症状)为研究对象,并使用临床试验随机编码表分为对照组和干预组,各40例。对照组采用护臀霜进行皮肤护理,干预组采用黄油膏进行皮肤护理,比较2组经护理后的肛周皮肤损伤等情况。结果干预组的肛周皮肤损伤发生率低于对照组,相关并发症总发生率也低于对照组,数据比较均有统计学差异(P均<0.05)。干预组的皮肤损伤时间短于对照组,皮肤不适感缓解时间早于对照组,医疗费用低于对照组,数据比较均有统计学差异(P均<0.05)。干预组的皮肤损伤面积、渗液量及外观评分均低于对照组,数据比较均有统计学差异(P均<0.05)。结论对伴有腹泻的重症患者外涂黄油膏,不仅能预防肛周皮肤损伤的发生,缩短肛周皮肤损伤时间,促进皮肤不适感快速缓解,还能减轻皮肤损伤的严重程度,降低相关并发症发生率,并且医疗费用较低。Objective To evaluate the effect of external application of yellow ointment on preventing perianal skin damage caused by diarrhea in critically ill patients.Methods A total of 80 critically ill patients(with diarrhea symptoms)admitted to the intensive care unit of Maoming Hospital of Traditional Chinese Medicine from March 2021 to March 2023 were selected as the study subjects,and a clinical trial random coding table was used to divide them into a control group and an intervention group,with 40 cases in each group.The control group received skin care with hip protection cream,while the intervention group received skin care with yellow ointment.The perianal skin damage and other conditions after nursing were compared between the two groups.Results The incidence of perianal skin damage in the intervention group was lower than that in the control group,and the total incidence of related complications in the intervention group was also lower than that in the control group.The data comparison showed statistical differences(both P<0.05).The intervention group had shorter skin damage time and earlier relief of skin discomfort compared to the control group,and the medical expenses in the intervention group were lower than those in the control group.The data comparison showed statistical differences(all P<0.05).The skin damage area,exudate volume,and appearance score of the intervention group were lower than those of the control group,and the data comparison showed statistical differences(all P<0.05).Conclusion Applying yellow ointment to critically ill patients with diarrhea can not only prevent the occurrence of perianal skin damage,shorten the time of perianal skin damage,promote rapid relief of skin discomfort,but also reduce the severity of skin damage,reduce the incidence of related complications,and have lower medical costs.
