腹腔镜髂耻韧带悬吊术、骶韧带悬吊术治疗盆腔器官脱垂的对照研究

摘　　要：目的探讨盆腔器官脱垂患者经腹腔镜髂耻韧带悬吊术、骶韧带悬吊术治疗后,盆腔器官脱垂量化测量值、生活质量等的差异性。方法方便选取2021年9月—2023年1月在龙岩市第二医院就诊的41例盆腔器官脱垂患者为研究对象,按随机数表法分观察组(髂耻韧带悬吊术组)21例和对照组(骶韧带悬吊术组)20例。比较两组患者的手术时间、术中出血量及术后6个月盆腔器官脱垂量化测量值等指标。结果观察组患者的手术时间为(72.34±9.20)min,住院时间为(5.46±0.52)d,均短于对照组的(85.50±13.16)min、(6.63±0.70)d,差异有统计学意义(t=3.726、6.096,P均<0.05)。术后6个月,观察组患者的盆腔器官脱垂量化测量值、盆底障碍影响简易问卷-7评分和盆底功能障碍问卷简表-20评分低于对照组,盆腔器官脱垂/尿失禁性功能问卷表-12评分高于对照组,差异有统计学意义(P均<0.05)。结论与骶韧带悬吊术相比,髂耻韧带悬吊术能够缩短盆腔器官脱垂患者的手术时间、住院时长,改善生活质量。Objective To investigate the difference of quantitative measurement value and quality of life in patients with pelvic organ prolapse treated by laparoscopic iliopectineal ligament suspension and sacral ligament suspension.Methods A total of 41 patients hospitalized in the Gynecology Department of Longyan Second Hospital from Septem⁃ber 2021 to January 2023 were conveniently selected as the study objects and were divided into observation group(il⁃iopectineal ligament suspension group)with 21 cases and control group(sacral ligament suspension group)with 20 cases according to random number table method.The operation time,intraoperative blood loss and quantitative mea⁃surement of pelvic organ prolapse 6 months after operation were compared between the two groups.Results The opera⁃tion time[(72.34±9.20)min]and hospitalization time[(5.46±0.52)d]in the observation group were shorter than those in the control group(85.50±13.16)min and(6.63±0.70)d,and the differences were statistically significant(t=3.726,6.096,both P<0.05).At 6 months after operation,the quantitative measurement of pelvic organ prolapse,Pelvic Floor Impact Questionnaire Short Form 7 and Pelvic Floor Distress Inventory Short Form 20 scores in observation group were lower than those in control group,and the differences were statistically significant(all P<0.05).The score of Pelvic Organ Prolapse/Urinary Incontinence Function Questionnaire-12 in observation group was higher than that of con⁃trol group,and the difference was statistically significant(P<0.05).Conclusion Comparing with sacral ligament sus⁃pension,iliopectineal ligament suspension is significantly better for patients with pelvic organ prolapse in terms of shortening operation time,length of hospital stay and improving quality of life.
