摘 要:目的探讨参芪降糖颗粒联合甲钴胺注射液对2型糖尿病周围神经病变患者人髓磷脂碱性蛋白(My-elin Basic Protein,MBP)、人脑源性神经营养因子(Brain-derived Neurotrophic Factor,BDNF)、神经生长因子(Nerve Growth Factor,NGF)水平的影响。方法选取2019年5月—2020年12月厦门前埔医院收治的62例2型糖尿病周围神经病变患者为研究对象,通过随机数表法分为西药组和观察组,各31例。西药组肌内注射甲钴胺注射液治疗,观察组在西药组基础上加用参芪降糖颗粒治疗。比较两组临床效果和药物安全性,治疗前后的中医证候积分、神经传导速度和MBP、BDNF、NGF水平。结果观察组的总有效率(96.77%)高于西药组的74.19%,差异有统计学意义(χ^(2)=6.369,P<0.05)。治疗3个月后两组各中医证候积分均降低明显,相比西药组,观察组更低,差异有统计学意义(P均<0.05)。治疗3个月后两组神经传导速度提升明显,相比于西药组,观察组更高,差异有统计学意义(P均<0.05)。治疗3个月后观察组MBP水平低于西药组,观察组BDNF、NGF高于西药组,差异有统计学意义(P均<0.05)。西药组和观察组不良反应发生率分别为6.45%和9.68%,差异无统计学意义(P>0.05)。结论对2型糖尿病周围神经病变患者应用参芪降糖颗粒联合甲钴胺注射液治疗效果显著,明显降低患者中医证候积分及MBP水平,提升神经传导速度及BDNF、NGF水平。Objective To investigate the effects of Shenqi Jiangtang granules combined with methylcobalamin injec-tion on the levels of human myelin basic protein(MBP),human brain-derived neurotrophic factor(BDNF),and nerve growth factor(NGF)in patients with peripheral neuropathy in type 2 diabetes mellitus.Methods 62 patients with type 2 diabetic peripheral neuropathy admitted to Xiamen Qianpu Hospital from May 2019 to December 2020 were se-lected as study objects and divided into the Western medicine group and the observation group by randomized numeri-cal table method,with 31 cases in each group.The Western medicine group was treated with intramuscular injection of methylcobalamin injection,and the observation group was treated with Shenqi Jiangtang granules on the basis of the Western medicine group.The clinical effects and drug safety of the two groups were compared,as well as the TCM evi-dence points,nerve conduction velocity and MBP,BDNF and NGF levels before and after treatment.Results The total effective rate of the observation group(96.77%)was higher than that of the western medicine group(74.19%),and the difference was statistically significant(χ^(2)=6.369,P<0.05).After 3 months of treatment,all TCM evidence points of both groups were reduced significantly,and compared with the western medicine group,the observation group was lower,and the differences were statistically significant(all P<0.05).Nerve conduction velocity was elevated signifi-cantly in both groups after 3 months of treatment,and compared with the western medicine group,it was higher in the observation group,and the differences were statistically significant(all P<0.05).MBP level of the observation group was lower than that of the western drug group after 3 months of treatment,BDNF and NGF of the observation group were higher than that of the western drug group,and the differences were statistically significant(all P<0.05).The inci-dence rates of adverse reactions in the western medicine group and observation group were 6.45%and 9.68%,respec-tively,a
