摘　　要：目的 构建新型产时会阴按摩法结合控制胎头娩出速度助产方案。方法 结合文献及临床经验,构建新型产时会阴按摩法结合控制胎头娩出速度助产方案。2021年10—12月邀请来自郑州市的12名产科护理专家进行德尔菲专家咨询。结果 共实施专家咨询2轮,问卷均有效回收;2轮咨询各级指标肯德尔和谐系数分别为0.242~0.488、0.126~0.280,差异有统计学意义(P<0.01);第2轮咨询各指标变异系数为0.06~0.22,满分率为33.3%~91.7%;2轮咨询专家权威系数分别为0.85、0.89。最终确定的构建新型产时会阴按摩法结合控制胎头娩出速度助产方案包括一级指标4个、二级指标9个和三级指标49个。结论 构建的新型产时会阴按摩法结合控制胎头娩出速度助产方案具有科学性、针对性、实用性及可行性。Objective To establish a new midwifery scheme of perineal massage during labor combined with control of fetal head delivery speed.Methods According to the literature and clinical experience,a new scheme of perineal massage combined with control of fetal head delivery speed was established.From October to December 2021,12 obstetric care experts from Zhengzhou were invited to conduct Delphi expert consultation.Results Two rounds of expert consultation were carried out,and the questionnaires were effectively recovered.The Kendall harmony coefficients of all indexes were 0.242-0.488 and 0.126-0.280,respectively,with statistical significance(P<0.01).The variation coefficient of each index in the second round of consultation ranged from 0.06 to 0.22,and the full score rate ranged from 33.3%to 91.7%.The authority coefficients of the experts in the two rounds of consultation were 0.85 and 0.89 respectively.The final plan of constructing the new perineal massage method combined with controlling the speed of fetal head delivery included 4 first-level indicators,9 second-level indicators and 49 third-level indicators.Conclusion The new intrapartum perineal massage combined with control of fetal head delivery speed is scientific,targeted,practical and feasible.
