超声引导下针刀神经触激术联合青藤碱注射液介入治疗梨状肌综合征的效果

摘　　要：目的 探讨超声引导下针刀神经触激术联合青藤碱注射液介入治疗对梨状肌综合征(PS)患者的效果。方法 将符合标准的30例PS患者随机分为对照组和联合组,对照组接受塞来昔布胶囊、甲钴胺片联合祖师麻片治疗,联合组接受超声引导下针刀神经触激术联合青藤碱注射液介入治疗。比较两组疼痛程度、腰背肌功能、疼痛相关神经因子水平。结果 治疗后5、10 d,两组视觉模拟量表评分均下降,且联合组低于对照组(P<0.05);治疗后5、10 d,两组日本骨科协会评估治疗分数各维度评分均上升,且联合组高于对照组(P<0.05);两组5-羟色胺和前列腺素E2表达均降低,且联合组低于对照组(P<0.05)。结论 应用超声引导下针刀神经触激术联合青藤碱注射液介入治疗可降低PS患者疼痛,减少疼痛相关因子的释放,提高腰背肌功能。Objective To explore the effect of ultrasound-guided needle knife nerve stimulation combined with sinomenine injection intervention treatment on patients with piriformis syndrome(PS).Methods Thirty patients with PS who met the criteria were randomly divided into control group and combination group.The control group was treated with celecoxib capsules,mecobalamin tablets and zushima tablets,while the combination group was treated with ultrasound-guided needle knife nerve stimulation combined with sinomenine injection intervention.The pain level,lumbar and dorsal muscle function,pain related neurotrophic factors levels of the two groups were compared.Results Visual analogue scale scores of both groups decreased at 5 and 10 days after treatment,and the combined group was lower than the control group(P<0.05).The Japanese Orthopaedic Association scores of both groups increased at 5 and 10 days after treatment,and the combined group was higher than the control group(P<0.05).The expressions of 5-hydroxytryptamine and prostaglandin E2 were decreased in both groups,and the combined group was lower than the control group(P<0.05).Conclusion Ultrasound-guided needle knife nerve stimulation combined with sinomenine injection can reduce pain of patients with PS,reduce the release of pain related factors,improve lumbar and dorsal muscle function.
