升降汤治疗肝郁脾弱型慢性萎缩性胃炎的效果

摘　　要：目的 探讨肝郁脾弱型慢性萎缩性胃炎患者采用升降汤治疗的效果。方法 采用前瞻性随机对照研究方法,选取2022年1月至2023年5月新乡医学院第一附属医院收治的90例肝郁脾弱型慢性萎缩性胃炎患者为研究对象,按随机数表法分为对照组(45例)与治疗组(45例),对照组采用常规西药治疗,治疗组在对照组基础上采用升降汤治疗,两组均连续治疗4周。观察两组临床疗效,对比两组治疗前后中医证候积分、胃镜表现、胃黏膜活检病理萎缩改善情况。结果 治疗组总有效率(95.56%)高于对照组(80.00%)(P<0.05);治疗后中医证候积分[胃脘胀满和(或)隐痛、纳少嗳气、胸闷胀满、嘈杂反酸、倦怠乏力、大便溏泄或便秘]治疗组低于对照组(P<0.05);治疗后治疗组胃镜下萎缩性胃炎表现积分低于对照组(P<0.05);两组治疗后胃黏膜活检腺体萎缩、肠上皮化生、慢性炎症评分均低于治疗前,治疗组低于对照组(P<0.05)。结论 升降汤治疗肝郁脾弱型慢性萎缩性胃炎患者效果显著,可明显改善慢性萎缩性胃炎患者中医证候和胃镜下萎缩性胃炎表现,在一定程度上可逆转胃黏膜萎缩,延缓疾病进展。Objective To explore the effect of Shengjiang decoction on chronic atrophic gastritis patients with liver depression and spleen weakness.Methods A prospective randomized controlled study was conducted.From January 2022 to May 2023,90 patients with chronic atrophic gastritis with liver depression and spleen weakness in the First Affiliated Hospital of Xinxiang Medical University were selected as the research objects.According to the random number table method,they were divided into control group(45 cases)and treatment group(45 cases).The control group was treated with conventional western medicine,and the treatment group was treated with Shengjiang decoction on the basis of the control group.Both groups were treated for 4 consecutive weeks.The clinical efficacy of the two groups was observed,and the improvement of traditional Chinese medicine syndrome scores,gastroscopy performance and gastric mucosal biopsy pathological atrophy before and after treatment were compared between the two groups.Results The total effective rate of the treatment group(95.56%)was higher than that of the control group(80.00%)(P<0.05).After treatment,the traditional Chinese medicine syndrome score[(epigastric distension and(or)dull pain,low belching,chest tightness,noisy acid reflux,fatigue,loose stool or constipation)]of the treatment group was lower than that in the control group(P<0.05).The atrophic gastritis performance scores under gastroscopy of the treatment group after treatment were lower than those of the control group(P<0.05).The gland atrophy,intestinal metaplasia and chronic inflammation scores of the two groups after treatment were lower than those before treatment,and the treatment group was lower than the control group(P<0.05).Conclusion Shengjiang decoction has significant effect on chronic atrophic gastritis patients with liver depression and spleen weakness,which can significantly improve the traditional Chinese medicine syndromes and the performance of atrophic gastritis under gastroscopy in patients with chronic atrop
