摘 要:目的检测原发性干燥综合征(primary Sjögren syndrome,pSS)患者唾液腺中ERdj5、XBP1的表达水平,分析其与唾液腺损伤程度和口腔症状相关性,探讨其在pSS发病中的意义。方法随机选取2020年1月—2021年6月西安市红会医院和青海省人民医院风湿免疫科pSS患者共60例作为病例组,随机选取同期青海省人民医院健康者20例作为对照组。采用反转录-聚合酶链反应和蛋白免疫印迹法分别检测唾液腺组织中ERdj5、XBP1 mRNA和蛋白表达,分析其与组织病理、唾液流率和口干程度及临床检验指标的相关性。结果病例组ERdj5、XBP1的mRNA和蛋白的表达均高于对照组(P<0.001),病例组ERdj5与XBP1 mRNA的表达呈正相关(r_(s)=0.936,P<0.001)。病例组ERdj5 mRNA和XBP1 mRNA的表达与唇腺组织病理分级(r_(s)=0.344,P<0.001;r_(s)=0.401,P<0.001)、口干程度(r_(s)=0.683,P<0.001;r_(s)=0.730,P<0.001)、抗Ro/SSA抗体(r_(s)=0.363,P<0.001;r_(s)=0.350,P<0.001)、抗La/SSB抗体(r_(s)=0.506,P<0.001;r_(s)=0.471,P<0.001)、欧洲抗风湿病联盟干燥综合征疾病活动指数(r_(s)=0.782,P<0.001;r_(s)=0.865,P<0.001)呈正相关,与唾液流率(r_(s)=−0.445,P<0.001;r_(s)=−0.526,P<0.001)呈负相关。结论pSS患者唾液腺中ERdj5和XBP1表达增高,唾液腺组织中ERdj5和XBP1的表达与疾病活动度、抗Ro/SSA和抗La/SSB自身抗体的产生、唾液腺的损伤程度以及患者的口腔症状具有相关性。Objective To detect the expression levels of ERdj5 and XBP1 in the salivary glands of patients with primary Sjogren syndrome(pSS),and analyze the correlation of the expression levels of ERdj5 and XBP1 with salivary gland damage and oral symptoms to explore their significance in the pathogenesis of pSS.Methods A total of 60 pSS patients in the departments of rheumatology and immunology of Xi’an Honghui Hospital and Qinghai Provincial People’s Hospital between January 2020 and June 2021were randomly selected as the case group,and 20 healthy participants in Qinghai Provincial People’s Hospital in the same period were randomly selected as the control group.The mRNA and protein expression levels of ERdj5 and XBP1 in salivary gland tissues were detected by rever_(s)e transcription-polymerase chain reaction and Western blot,respectively,and their correlation with histopathology,salivary flow rate,dry mouth degree,and clinical test indicator_(s) were analyzed.Results The mRNA and protein expression levels of ERdj5 and XBP1 in the case group were higher than those in the control group(P<0.001).The ERdj5 mRNA expression was positively correlated with the XBP1 mRNA expression in the case group(r_(s)=0.936,P<0.001).The expression levels of ERdj5 mRNA and XBP1 mRNA in the case group were positively correlated with the pathological grade of labial gland(r_(s)=0.344,P<0.001;r_(s)=0.401,P<0.001),dry mouth degree(r_(s)=0.683,P<0.001;r_(s)=0.730,P<0.001),anti-Ro/SSA antibody(r_(s)=0.363,P<0.001;r_(s)=0.350,P<0.001),anti-La/SSB antibody(r_(s)=0.506,P<0.001;r_(s)=0.471,P<0.001),and European League Against Rheumatism Sjogren’s Syndrome Disease Activity Index(r_(s)=0.782,P<0.001;r_(s)=0.865,P<0.001),and were negatively correlated with salivary flow rate(r_(s)=-0.445,P<0.001;r_(s)=-0.526,P<0.001).Conclusion The expression levels of ERdj5 and XBP1 in salivary glands are increased in pSS patients,and they are related to disease activity,anti-Ro/SSA and anti-La/SSB autoantibodies,damage degree of salivary gland and oral symptoms o
