刘磊基于“金水相生”理论治疗非小细胞肺癌术后咳嗽经验

摘　　要：肺癌术后咳嗽在临床治疗上存在易复发、长期疗效欠佳的问题,中医药治疗具有辨证论治,治病求因的特点。文章总结了刘磊副教授治疗肺癌术后咳嗽的临证经验,刘磊副教授认为肺癌术后咳嗽病位主在肺肾,在五行对应关系中,肺属金,肾属水,金为水之母,若金虚,母病及子,日久可及肾,终致肺肾两虚。其病机为肺肾阴虚,阴虚生热,虚火上炎,肺失肃降,气机上逆,治疗上依据“金水相生”理论,以滋养肺肾、止咳平喘为主。结合团队前期理论研究及多年临床经验,刘磊副教授认为,中医学在治疗肺癌术后咳嗽方面疗效较好,对肺癌术后肺肾阴虚型咳嗽患者使用百合固金汤合生脉散加减治疗,可有效改善其临床症状,提高其免疫功能,进而提高其生存质量。附临床验案1例予以佐证。The cough after lung cancer surgery is easy to recur and the long-term curative effect is not good.The traditional Chinese medicine treatment is characterized by syndrome differentiation and cause of treatment.The article summarized associate Professor Liu Lei's clinical experience in the treatment of cough after lung cancer surgery,and the associate professor Liu Lei thought that the cough after lung cancer surgery is mainly in lung and kidney,the lung is gold,the kidney is water,and gold is the mother of water.Mother's disease and son,can reach the kidney for a long time,and eventually the lung and kidney deficiency,its pathogenesis is lung and kidney Yin deficiency,Yin deficiency heat,deficiency fire on inflammation,lung loss,Qi machinery on the reverse,treatment according to the “gold and water phase” theory,to nourish lung and kidney,cough and asthma.Combined with the team's previous theoretical research and many years of clinical experience,Associate Professor Liu Lei believes that traditional medicine has a good effect in the treatment of cough after lung cancer surgery,and the addition and subtraction of Baihe Gujin Decoction and Shengmai Powder for patients with lung and kidney Yin deficiency cough after lung cancer surgery can effectively improve their clinical symptoms,improve their immune function,and thus improve their quality of life.One clinical case is attached as evidence.
