髌内侧滑膜皱襞松解术与全膝关节置换术治疗膝骨性关节炎的有效性和安全性对比

摘　　要：目的 探讨髌内侧滑膜皱襞松解术与全膝关节置换术治疗膝骨性关节炎的有效性和安全性。方法 选择2022年1—12月本院收治的70例膝骨性关节炎患者作为研究对象,随机分为对照组和观察组两组,每组各35例。对照组实施全膝关节置换术,观察组实施髌内侧滑膜皱襞松解术。比较两组治疗前后VAS评分、HSS评分、Lysholm评分以及WOMAC评分高低,比较临床治疗效果、并发症发生情况以及患者满意度。结果 治疗后,观察组VAS以及WOMAC评分均低于对照组,差异均具有统计学意义(P<0.05);观察组HSS以及Lysholm评分均高于对照组,差异均具有统计学意义(P<0.05),以上结果均表明膝关节功能恢复较好。观察组总有效率为100.00%,明显好于对照组的80.00%,差异具有统计学意义(P<0.05)。观察组不良反应发生率为2.86%,低于对照组的22.86%,差异具有统计学意义(P<0.05)。观察组患者总满意率为100.00%,高于对照组的82.86%,差异具有统计学意义(P<0.05)。结论 对于膝骨性关节炎患者实施髌内侧滑膜皱襞松解术与全膝关节置换术治疗均有一定的治疗效果,但实施髌内侧滑膜皱襞松解术能够显著减轻患者的疼痛,改善患者的膝关节功能,提高临床治疗效果,降低并发症的发生率并提高患者的满意度。Objective To probe into the efficacy and safety of medial patellar synovial plica release surgery and total knee arthroplasty in the treatment of knee osteoarthritis.Methods 70 patients suffering from knee osteoarthritis who were admitted to our hospital from January to December 2022 were selected as the research subjects and they were randomly divided into the control group and the observation group,with 35 cases in each group.The control group underwent total knee replacement surgery,while the observation group underwent medial patellar synovial fold release surgery.The VAS score,HSS score,Lysholm score,and WOMAC score before and after treatment between two groups were compared,and the clinical treatment effect,incidence of complications,and patient satisfaction were compared.Results After treatment,the VAS and WOMAC scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).The HSS and Lysholm scores of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The above results indicated that knee joint function had recovered well.The total effective rate of the observation group was 100%,which was significantly better than the 80%of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 2.86%,it was lower than 22.86%of the control group,and the difference was statistically significant(P<0.05).The total satisfaction rate of patients in the observation group was 100%,which was higher than the 82.86%in the control group,and the difference was statistically significant(P<0.05).Conclusions Both medial patellar synovial fold release and total knee arthroplasty have certain therapeutic effect on patients with knee osteoarthritis.However,medial patellar synovial fold release surgery can significantly reduce patient pain,improve knee joint function,enhance clinical treatment effectiveness,reduce
