补肺益肾方联合呼吸肌功能锻炼治疗老年慢性阻塞性肺疾病合并肺动脉高压患者的疗效观察

摘　　要：目的:观察用补肺益肾方联合呼吸肌功能锻炼治疗老年慢性阻塞性肺疾病(COPD)合并肺动脉高压(PH)的临床效果。方法:将116例老年COPD合并PH患者随机分为对照组和观察组,各58例。两组患者均给予西医常规治疗,对照组在此基础上给予呼吸肌功能锻炼;观察组在对照组的基础上给予补肺益肾方口服,连续治疗2周。比较两组中医证候积分、肺功能、平均肺动脉压(mPAP)、Borg呼吸指数、6 min步行测试(6MWT)距离、血清因子水平。结果:观察组的临床总有效率、肺通气功能指标高于对照组;中医证候积分、mPAP、Borg呼吸指数及血清红细胞压积、C反应蛋白、脑钠肽、血浆D-二聚体水平低于对照组,6MWT距离长于对照组(P<0.05)。结论:补肺益肾方联合呼吸肌功能锻炼利于减轻老年COPD合并PH患者的临床症状,改善肺功能、运动耐力和血液流变学,降低肺动脉压和炎症反应。Objective:To observe the clinical efficacy of Bufei Yishen Formular combined with respiratory muscle functional exercise in the treatment of elderly chronic obstructive pulmonary disease(COPD)complicated with pulmonary hypertension(PH).Methods:One hundred and sixteen elderly patients with COPD complicated with PH were randomly divided into a control group and an observation group with 58 cases each.Both groups were given conventional treatment with Western medicine,and the control group was additionally given respiratory muscle functional exercise and the observation group was given Bufei Yishen Formular orally on the basis of the control group.The treatment was lasted for 2 weeks.Chinese medical syndrome scores,lung function,mean pulmonary artery pressure(mPAP),Borg respiratory index,6-minute walking test(6MWT)distance,and levels of serum factors were compared between the two groups.Results:The total clinical efficacy and indexes of pulmonary ventilation function were higher,the scores of Chinese medicine syndrome,mPAP,Borg respiratory index and hematocrit,the levels of C-reactive protein,brain natriuretic peptide and D-dimer were lower,and the distance of the 6MWT was longer in the observation group than the control group(P<0.05).Conclusion:Bufei Yishen Formular combined with respiratory muscle functional exercise is of benefit to reducing clinical symptoms,improving lung function,exercise endurance and blood rheology,and decreasing pulmonary arterial pressure and inflammatory response in elderly patients with COPD complicated with PH.
