城乡居民医保整合与农村老年人相对贫困风险的影响研究

摘　　要：文章基于可行能力理论视角,利用中国健康与养老追踪调查(CHARLS)2013年和2018年数据构造两期平衡面板数据,以贫困脆弱性衡量农村老年人相对贫困风险并运用PSM-DID模型和逐步中介模型实证研究了城乡居民医保整合对农村老年人相对贫困风险的影响与作用机制。研究发现,城乡居民医保整合显著降低了农村老年人相对贫困风险,使其将来陷入相对贫困的概率减少3.89%;异质性分析表明城乡居民医保整合对相对贫困风险的抑制效果在收入较高或遭受大病冲击的农村老年人群体中更好;机制检验表明改善储蓄和健康水平是城乡居民医保整合影响农村老年人相对贫困风险的重要渠道。基于此,文章围绕可行能力从加快推进城乡基本公共服务均等化、完善农村老年人相对贫困动态监测和帮扶机制、适时调整老年人医保待遇等方面提出了政策建议。Based on the perspective of feasible ability theory,this article constructs two periods of balanced panel data using the China Health and Retirement Tracking Survey(CHARLS)data from 2013 and 2018.The relative poverty risk of rural elderly is measured by poverty vulnerability,and the PSM-DID model and stepwise mediation model are used to empirically study the impact and mechanism of urban-rural resident medical insurance integration on the relative poverty risk of rural elderly.Research has found that the integration of medical insurance for urban and rural residents significantly reduces the relative poverty risk of rural elderly people,reducing their probability of falling into relative poverty in the future by 3.89%;heterogeneity analysis shows that the integration of medical insurance for urban and rural residents has a better inhibitory effect on relative poverty risk in rural elder⁃ly populations with higher incomes or those affected by major diseases;mechanism testing shows that improving savings and health levels is an important channel for the integration of medical insurance for urban and rural residents to affect the relative poverty risk of rural elderly people.Based on this,the article proposes policy recommendations around feasible capabilities,such as accel⁃erating the equalization of basic public services between urban and rural areas,improving the dynamic monitoring and assistance mechanism for relative poverty among rural elderly,and timely adjusting the medical insurance benefits for the elderly.
