摘 要:目的探讨3D Slicer三维影像重建技术分析骨半规管及前庭导水管的解剖学意义。方法收集40例(男女各20例)无内耳器质性病变的病人的80侧颞骨的高分辨率计算机断层扫描(HRCT)数据,应用3D Slicer软件进行三维重建,并测量内听道重要解剖结构三维空间定位数据。结果三维模型显示,内听道后唇呈椭圆形,后缘锐利,位置固定,较易识别。选择三维模型内听道后唇为P点,男性P点至前庭导水管的距离为(9.412±1.687)mm、至后半规管的距离为(10.422±1.701)mm;内听道的长度为(9.847±0.870)mm。女性P点至前庭导水管的距离为(9.068±1.133)mm、至后半规管的距离为(9.858±1.089)mm;内听道的长度为(8.727±0.964)mm。男性和女性上述测量结果均无统计学差异(P>0.05)。结论内听道后壁至前庭导水管的距离比到后半规管更短,提示术中磨除内听道后壁时更容易导致前庭导水管的损伤。前庭导水管外口和内淋巴囊靠近内听道后壁的硬脑膜,提示术中剪开硬脑膜时应避免造成损伤。Objective To investigate the anatomical significance of analysis of bone semicircular canal and vestibular aqueduct by three-dimensional(3D)image reconstruction using 3D Slicer software.Methods The high-resolution computed tomography(HRCT)data of the temporal bone of 40 patients(80 sides;20 males and 20 females)without organic inner ear lesions were collected.The 3D Slicer software was used for 3D reconstruction,and the 3D spatial positioning data of important anatomical structures of the internal auditory canal were measured.Results The 3D images showed that the posterior lip of the internal auditory canal was oval,with a sharp posterior edge and a fixed position,which was easy to identify.The posterior lip of the internal auditory canal was selected as the P point in the 3D images.The distances from the P point to the vestibular aqueduct and the posterior semicircular canal were(9.412±1.687)mm and(10.422±1.701)mm in male patients,respectively;the length of the internal auditory canal was(9.847±0.870)mm in male patients.The distances from the P point to the vestibular aqueduct and the posterior semicircular canal were(9.068±1.133)mm and(9.858±1.089)mm in female patients,respectively;the length of the internal auditory canal was(8.727±0.964)mm in female patients.There was no significant difference in the above measurements between male and female patients(P>0.05).Conclusions The distance from the posterior wall of the internal auditory canal to the vestibular aqueduct was shorter than that to the posterior semicircular canal,suggesting that the removal of the posterior wall of the internal auditory canal is more likely to cause damage to the vestibular aqueduct.The external opening of the vestibular aqueduct and the endolymphatic sac are close to the dura mater of the posterior wall of the internal auditory canal,suggesting that the injury to these structures should be avoided when shearing the dura mater.
