某院2021~2022年住院患者抗菌药物使用分析

摘　　要：目的 分析我院2021~2022年抗菌药物的使用情况以及常见细菌的耐药现状,以期改善我院抗菌药物的使用情况,促进抗菌药物的合理使用。方法 通过医院信息系统(HIS)调取我院2021~2022年住院患者的抗菌药物使用数据,计算抗菌药物使用率、使用强度和使用频度(DDDs)。通过逸曜合理用药管理系统调取2021~2022年住院患者抗菌药物使用前药敏测试送检结果,计算出全年各科室药敏测试送检率和测试结果。结果 相比2021年,2022年的抗菌药物使用率无明显改变,但是大部分科室抗菌药物使用强度和DDDs明显增加。在DDDs排名前10的抗菌药物中,90%的药物DDDs升高,而抗菌药物的用药前送检率有所下降。药敏测试方面,除大肠埃希菌对常用抗菌药物耐药率变化不大外,粪肠球菌和无乳链球菌的耐药率均下降。结论 我院住院患者的抗菌药物使用基本合理,2022年较2021年住院患者的抗菌药物的使用强度和使用频度均升高,但用药前送检率下降。建议我院加强耐药监测和指导以及对抗菌药物使用人员的教育培训,促进抗菌药物的合理使用,减少细菌耐药性的发展。OBJECTIVE To analyze the use of antibacterial drugs in our hospital from 2021 to 2022 and the current situation of drug resistance of common bacteria in order to improve the use of antibacterial drugs and promote the rational use of antibacterial drugs in our hospital.METHODS The use data of the inpatients′ antibacterial drugs in our hospital from 2021 to 2022 were retrieved through the hospital information system(HIS),and the use rate,use intensity and frequency(DDDs) of the antibacterial drugs were calculated.The results of pre-use antimicrobial susceptibility testing of inpatients from 2021 to 2022 were retrieved through Yiyao′s rational drug use management system,and the annual susceptibility testing submission rates and test results for each department were calculated.RESULTS Compared with 2021,the utilization rate of antibacterial drugs in 2022 did not change significantly.However,the use intensity of antibacterial drugs and DDDs have increased significantly in most departments.Among the top 10 antibacterial drugs with DDDs,90% of the drugs′ DDDs increased.Meanwhile,the pre-use susceptibility testing submission rate of antibacterial drugs decreased.In terms of drug susceptibility testing,except for the unchanged resistance rate of Escherichia coli to commonly used antibacterial drugs,the drug resistance rates of Enterococcus faecalis and Streptococcus agalactiae to commonly used antibacterial drugs all decreased.CONCLUSION The use of antibacterial drugs in our hospital is basically reasonable.The frequency and intensity of antibacterial use by inpatients in 2022 increased compared with that in 2021,but the rate of pre-use susceptibility testing submission decreased.It is suggested that our hospital should strengthen the monitoring and guidance of drug resistance,and enhanced the education and training of antibacterial drug users,so as to promote the rational use of antibacterial drugs and reduce the development of bacterial drug resistance.
