摘　　要：目的本次研究主要探讨社区卫生服务中心药房发药差错的原因,并提出相应的预防对策,持续改进药房发药质量,降低差错发生率。方法研究选择2021年1—6月预防策略实施之前的2300张处方作为对照组,选择2021年7月-2022年1月预防策略实施之后的2300张处方为观察组,两组对比分析发药差错的原因。结果观察组的发药差错率显著低于对照组,差异有统计学意义(P<0.05);用药差错主要有发药错误、书写错误和剂量错误三大类。结论发药错误、书写错误和剂量错误是社区卫生服务中心药房发药差错的主要原因,可通过加大对药师培训力度、加大药品管理力度、加强医患沟通交流、弹性排班、对药品调配的发放流程进行完善、健全发药差错登记制度、加大药房药品养护管理力度等措施预防发药差错。Objective To explore causes of dispensing errors in community health service center pharmacies mainly,and proposes corresponding preventive measures to improve dispensing quality of pharmacies and reduce incidence of errors continuously.Methods The paper chose 2300 prescriptions before prevention from January 2021 to January 2022 as control group,and 2300 prescriptions in our hospital during the same period as observation group.Causes of medication errors were compared and analyzed between two groups.Results Medication error rate in observation group was significantly lower than control group,difference was statistically significant(P<0.05).Analysis showed,main causes of medication errors included 3 types of medication disp ensing errors,writing errors and dosage errors.Conclusion Dispensing errors,writing errors,and dosage errors are main causes of dispensing errors in community health service center pharmacies.To prevent the problems,it is necessary to take mearuses of strengthening pharmacist training,drug management,communication and exchange between doctors and patients,flexible scheduling,improving dispensing process of drugs and dispensing error registration system,increase pharmacy drug ma intenance management.
