老年股骨粗隆间骨折髓内与髓外固定疗效比较

摘　　要：目的对老年股骨粗隆间骨折患者在治疗时对比分析髓内固定以及髓外固定的临床效果。方法采用随机数字表法将2022年1月—2023年5月沭阳南关医院医治老年股骨粗隆间骨折患者75例共分为两个组别,包括对照组和观察组。对照组(37例)接受髓外固定治疗方法,观察组(38例)接受髓内固定治疗方法。比较两组临床相关指标、髋关节功能(Harris)评分、并发症发生情况。结果与对照组比较,观察组手术、术后患肢负重、住院以及骨折愈合时间比较短,术中出血量较少,差异有统计学意义(P<0.05)。观察组髋关节功能(Harris)评分比较高,差异有统计学意义(P<0.05)。观察组并发症发生率比较低,差异有统计学意义(P<0.05)。结论选择髓内固定治疗方法,对老年股骨粗隆间骨折患者效果较为显著,能够缩短临床各项所需时间,减少术中出血量,改善髋关节功能,减少并发症发生,在临床治疗方案中值得大范围推广与实施。Objective The results of intramedullary fixation and extramedullary fixation were compared and analyzed in the treatment of elderly patients with intertrochanteric femur fractures.Methods A total of 75 elderly patients with intertrochanteric fracture of the femur treated in Shuyang Nanguan Hospital during the period of January 2022 to May 2023 were divided into two groups,including the control group and the observation group,using randomized numerical table.In the control group,37 patients were treated with extramedullary fixation.In the observation group,38 patients were treated with intramedullary fixation.Comparison of clinical indicators,hip function(Harris)scores,and the occurrence of complications between the two groups.Results In the observation group,the operation,postoperative weight bearing of the affected limb,hospitalization and fracture healing time were shorter than that of the control group,and the intraoperative bleeding was less than that of the control group,with a more obvious difference between the two groups(P<0.05).Hip function(Harris)score of the observation group was higher than that of the control group,and there was a significant difference between the two groups(P<0.05).The complication rate of the observation group was lower than that of the control group,and there was a significant difference between the two groups(P<0.05).Conclusion Intramedullary fixation treatment has obvious therapeutic effect on elderly patients with intertrochanteric fracture of the femur,which can shorten the time needed for clinical procedures,reduce intraoperative bleeding,improve the function of the hip joint,and reduce the occurrence of complications,which is worthy of wide-scale popularization and implementation in the clinical treatment plan.
