膝骨性关节炎患者膝关节置换前后血常规变化对术后假体周围感染的影响

摘　　要：目的观察膝骨性关节炎患者行膝关节置换术前后血常规变化,并分析血常规变化对术后假体周围感染的影响,为膝骨性关节炎患者术后假体周围感染的防治提供参考。方法前瞻性选取2020年6月—2021年6月期间安阳钢铁集团有限责任公司职工总医院接收的行膝关节置换术的91例膝骨性关节炎患者为研究对象,于手术前、术后3 d以及术后1个月时对其进行白细胞计数(WBC)、血小板计数(PLT)和血红蛋白(HGB)水平检测,观察患者术后1个月假体周围感染发生情况,比较术后有无假体周围感染与患者术前、术后WBC、PLT和HGB水平的关系。结果患者经膝关节置换术后,发生假体周围感染者3例,感染率为3.30%。术后3 d以及术后1个月时,膝骨性关节炎患者WBC、PLT和HGB水平均高于术前,差异有统计学意义(P<0.05)。术后假体周围感染患者术前、术后3 d及术后1个月时WBC、PLT和HGB水平均高于未感染患者,差异有统计学意义(P<0.05)。结论膝骨性关节炎患者术前血常规异常且术后血常规数值较术前异常升高,则患者术后发生假体周围感染的风险较高。Objective To observe the changes of blood routine in patients with knee osteoarthritis before and after knee replacement,and analyze the influence of blood routine changes on periprosthetic joint infection,so as to provide reference for the prevention and treatment of periprosthetic joint infection in patients with knee osteoarthritis.Methods A prospective study was conducted on 91 patients with knee osteoarthritis who underwent knee replacement in Anyang Iron and Steel Group Co.,Ltd.Employee General Hospital from June 2020 to June 2021.The levels of white blood cell count(WBC),platelet count(PLT)and hemoglobin(HGB)were measured before operation,3 days after operation and 1 month after operation.The incidence of periprosthetic joint infection was observed 1 month after operation.The relationship between periprosthetic joint infection and the levels of WBC,PLT and HGB before and after operation was compared.Results After knee replacement,3 patients had periprosthetic joint infection,the infection rate was 3.30%.The levels of WBC,PLT and HGB in patients with knee osteoarthritis at 3 days and 1 month after operation were higher than those before operation,with a statistical significant difference(P<0.05).The levels of WBC,PLT and HGB in patients with periprosthetic joint infection before operation,at 3 days after operation and at 1 month after operation were higher than those of patients without periprosthetic joint infection,with a statistical significant difference(P<0.05).Conclusion Patients with knee osteoarthritis have abnormal blood routine before operation and the blood routine value after operation is abnormally higher than that before operation,so the risk of periprosthetic joint infection is higher.
