重症创伤病人家属疾病不确定感现状及影响因素研究

摘　　要：目的:调查ICU创伤病人家属疾病不确定感现状及影响因素。方法:采用便利抽样法纳入230名创伤病人家属作为研究对象,采用一般资料调查表、疾病不确定感家属量表、家庭关怀度量表和简易心理弹性量表进行调查,并用多元线性回归分析探讨家属疾病不确定感的影响因素。结果:ICU创伤病人家属疾病不确定感得分为(70.06±12.02)分,处于较高水平。Pearson分析显示,家庭关怀、心理弹性均与家属疾病不确定感呈负相关关系(P<0.01)。多元线性回归分析结果显示,病人首次转入ICU、病人意识昏迷可能是ICU创伤病人家属疾病不确定感的独立危险因素(P<0.05和P<0.01),家属家庭关怀程度好和心理弹性水平强可能是创伤病人家属疾病不确定感的独立保护因素(P<0.01)。结论:ICU创伤病人家属疾病不确定感水平较高,临床需重点关注首次转入ICU、病人意识昏迷、家庭关怀不足和心理弹性水平低的家属。Objective:To investigate the current status and influencing factors of illness uncertainty among family members of ICU trauma patients.Methods:A total of 230 family members of trauma patients were included as study subjects according to convenience sampling.The survey was carried out with a general information questionnaire,a family uncertainty scale,a family care scale and a simplified psychological resilience scale.Multiple linear regression analysis was used to explore the influencing factors of family members′illness uncertainty.Results:The uncertainty score of the family members of ICU trauma patients was(70.06±12.02)points,which was at a high level.Pearson analysis showed a negative correlation between family care,psychological resilience,and the uncertainty of family members(P<0.01).Multiple linear regression analysis showed that the first admissioion to ICU and the consciousness status of patients might be independent risk factors for the disease uncertainty in the family members of ICU trauma patients(P<0.05 and P<0.01),and good family care and strong psychological resilience might be independent protective factors for the disease uncertainty in the family members of trauma patients(P<0.01).Conclusions:The family members of ICU trauma patients have a high level of disease uncertainty,and more attention should be focused on patient′s family members of low psychological resilience,insufficient family care,as well as clinical severity,such as ICU admission and loss of consciousness.
