摘  要:目的基于甘肃省武威市急性呼吸道感染(acute respiratory infection,ARI)病例样本,分析人腺病毒(human adenovirus,HAdV)的感染情况以及基因分型。方法针对2021年1—8月采集的甘肃省武威市ARI病例的鼻咽拭子样本,使用荧光定量PCR(RT-PCR)对12种呼吸道病毒进行筛查;选取HAdV阳性样本进行病毒分离,普通PCR扩增分离株的Hexon基因Loop2区域以及3个结构蛋白(Penton base、Hexon、Fiber)全长,从GenBank数据库下载国内外各型别原型株以及代表株全基因组进行系统发育和一致性分析。结果本研究共收集ARI样本574份,共检出呼吸道病毒阳性标本233份,检出率位居前三位的分别是鼻病毒(15.16%,87/574)、人偏肺病毒(7.67%,44/574)和HAdV(7.32%,42/574)。574份样本中,男、女HAdV检出率差异无统计学意义;HAdV主要发生在0~1岁(10.61%,14/132);各月份均有检出,差异无统计学意义;从42份HAdV阳性标本中分离出28株阳性株,分别为HAdV-B(4株均为HAdV-3)和HAdV-C(包括7株P1H2F2、8株HAdV-1、9株Px1/Ps3H5F5)两个亚属,其中P1H2F2和Px1/Ps3H5F5为本研究基因重组模式。结论HAdV是甘肃省武威市2021年1—8月ARI病例中第三流行的病毒,感染型别以C亚属为主,其次为B亚属。Objective To analyze the prevalence and genotypes of human adenovirus(HAdV)in acute respiratory infection(ARI)cases in Wuwei,Gansu Province.Methods Nasopharyngeal swabs were collected from ARI cases in Wuwei,Gansu Province from January to August 2021.Real-time PCR was performed to detect 12 respiratory viruses in the samples.HAdV-positive samples were selected for virus isolation.The Loop2 region of Hexon gene and the full length of three structural proteins(Penton base,Hexon,Fiber)were amplified by PCR to identify the types of HAdV.The sequences of domestic and foreign prototype and representative strains of each type were downloaded from GenBank database for phylogenetic and consistency analysis.Results A total of 574 ARI samples were collected and 233 were positive for respiratory viruses.The top three viruses with the highest detection rates were rhinovirus(15.16%,87/574),human metapneumovirus(7.67%,44/574)and HAdV(7.32%,42/574).Among the 574 cases,no significant difference in the detection rates of HAdV was found between males and females.HAdV infection mainly occurred in children aged 0-1 year(10.61%,14/132)and could be detected in each month with no statistically significant difference.Twenty-eight HAdV strains were isolated from the 42 positive samples and classified into two subgenera,HAdV-B(all four strains were HAdV-3)and HAdV-C(including seven strains of P1H2F2,eight strains of HAdV-1 and nine strains of Px1/Ps3H5F5),of which P1H2F2 and Px1/Ps3H5F5 were the two genetic recombination modes identified in this study.Conclusions HAdV is the third most prevalent virus among ARI cases in Wuwei,Gansu Province from January to August 2021,and the predominant type is subgenus C,followed by subgenus B.
