多模式麻醉方式联合加速康复外科理念在结肠切除手术患者中的应用效果观察

摘　　要：目的:观察多模式麻醉方式联合加速康复外科理念在结肠切除手术患者中的应用效果。方法:将94例行结肠切除手术患者随机分为甲组和乙组,每组47例。甲组患者采用多模式麻醉方式联合加速康复外科理念干预,乙组患者采用加速康复外科理念干预,比较2组患者治疗情况(术后首次排气时间、术后首次排便时间、术中麻醉药物用量、术后追加镇痛药次数、住院时间、手术费用)、术后并发症发生情况,采用自我效能感评分、康复状态评分、健康行为评分对患者预后进行评定。结果:甲组患者术后首次排气时间、术后首次排便时间、住院时间、术后追加镇痛药次数、术中麻醉药物用量均短于或少于乙组(P<0.05),2组患者手术费用比较差异无统计学意义(P<0.05)。甲组患者术后并发症发生率明显低于乙组(P<0.05);干预前2组患者自我效能感评分、康复状态评分、健康行为评分比较差异无统计学意义(P>0.05),干预后甲组各项评分均高于乙组(P<0.05)。结论:多模式麻醉方式联合加速康复外科理念在结肠切除术中应用效果显著,可降低术后并发症发生率,改善其预后,值得临床推广应用。Objective To observe the application effect of multi-mode anesthesia combined with enhanced recovery after surgery(ERAS)concept in patients with colectomy.Methods Total 94patients with co-lectomy were randomly divided into group A and group B,with 47cases in each group.Patients in group A were treated with multi-mode anesthesia combined with ERAS concept intervention,while patients in group B were treated with ERAS concept intervention.The treatment conditions(first postoperative ex-haust time,first postoperative defecation time,intraoperative anesthetic dosage,postoperative additional an-algesics frequency,hospital stay,surgical costs),and postoperative complications were compared between the two groups.The prognosis was evaluated by self-efficacy score,rehabilitation status score,and health behavior score.Results The first postoperative exhaust time,first postoperative defecation time,hospital stay,postoperative additional analgesics frequency,and intraoperative anesthetic dosage of group A were shorter or less than those of group B(P<0.05).There was no significant difference in surgical costs be-tween the two groups(P<0.05).The incidence of postoperative complications in group A was significant-ly lower than that in group B(P<0.05).There was no significant difference in self-efficacy score,rehabili-tation status score,and health behavior score between the two groups before intervention(P>0.05).After intervention,the scores of group A were higher than those of group B(P<0.05).Conclusion Multi-mode anesthesia combined with ERAS concept has significant effect in colectomy,which can reduce the in-cidence of postoperative complications and improve the prognosis,and is worthy of clinical promotion and application.
