半夏泻心汤加减治疗寒热错杂型胃食管反流病患者的效果观察

摘　　要：目的观察半夏泻心汤加减治疗寒热错杂型胃食管反流病(GERD)患者的临床效果。方法40例寒热错杂型GERD患者,随机分为观察组和对照组,各20例。对照组给予基础对症治疗,观察组在对照组基础上给予半夏泻心汤加减治疗。比较两组患者临床疗效、食管动力指标、不良反应发生情况及复发情况。结果观察组总有效率90.00%较对照组的60.00%明显更高,差异有统计学意义(P<0.05)。治疗后,观察组患者的食管收缩波幅(65.79±15.42)mm Hg(1 mm Hg=0.133 kPa)、食管上括约肌静息压(65.33±15.42)mm Hg、食管下括约肌静息压(16.62±4.48)mm Hg均较对照组的(53.61±12.33)、(54.34±11.58)、(12.51±3.86)mm Hg显著升高,差异有统计学意义(P<0.05)。观察组不良反应发生率10.00%和复发率5.00%均较对照组的40.00%、35.00%更低,差异有统计学意义(P<0.05)。结论采用半夏泻心汤加减治疗寒热错杂型GERD患者,可以显著缓解反酸、烧心等临床症状,有效改善食管动力,疗效确切,可以在临床推广使用。Objective To observe the clinical effect of Banxia Xiexin decoction in treating gastroesophageal reflux disease(GERD)of cold-heat mixed type.Methods 40 patients with GERD of coldheat mixed type were randomly divided into an observation group and a control group,with 20 cases in each group.The control group was given basic symptomatic treatment,and the observation group was given Banxia Xiexin decoction on the basis of the control group.The clinical efficacy,esophageal motility index,occurrence of adverse reactions and recurrence were compared between the two groups.Results The total effective rate of 90.00% in the observation group was significantly higher than 60.00% in the control group,and the difference was statistically significant(P<0.05).After treatment,the esophageal contraction amplitude in the observation group was(65.79±15.42)mm Hg(1 mm Hg=0.133 kPa),the upper esophageal sphincter resting pressure was(65.33±15.42)mm Hg,and the lower esophageal sphincter resting pressure was(16.62±4.48)mm Hg,which were significantly higher than(53.61±12.33),(54.34±11.58)and(12.51±3.86)mm Hg of the control group.The difference was statistically significant(P<0.05).The incidence of adverse reactions and recurrence were 10.00% and 5.00% in the observation group,which were lower than 40.00% and 35.00% in the control group,and the difference was statistically significant(P<0.05).Conclusion The modified Banxia Xiexin decoction can significantly relieve the clinical symptoms such as acid regurgitation and heartburn in the patients with GERD of cold-heat mixed type,improve the patients'esophageal motility effectively,and the curative effect is definite,so it can be popularized and used in clinic.
