摘 要:目的 研究情景模拟演练对提高紧急剖宫产能力的作用,以及对母体和新生儿结局的影响。方法 选取2021年1—12月广州医科大学附属第三医院84名产科和手术室医护人员,按随机数字表法分为情景模拟演练组和常规培训组,每组各42名。常规培训组采用常规理论授课培训,情景模拟演练组在常规培训组基础上采用情景模拟演练。比较两组医护人员决定患者手术至进手术室时间、进手术室至手术开始时间、决定手术至胎儿娩出时间间隔(DDI)、新生儿及产妇相关结局。结果 情景模拟演练组DDI、决定手术至进手术室时间、进手术室至手术开始时间分别为(9.34±4.89)min、(3.57±3.45)min、(2.41±2.65)min;常规培训组DDI、决定手术至进手术室时间、进手术室至手术开始时间分别为(15.43±7.45)min、(7.32±5.67)min、(5.78±3.21)min。情景模拟演练组与常规培训组DDI、决定手术至进手术室时间、进手术室至手术开始时间比较差异均有统计学意义(t=4.621,2.373,4.892,均P<0.05)。两组紧急剖宫产中产妇的手术时间、剖宫产出血量、住院天数及新生儿脐动脉血pH值比较,差异均有统计学意义(均P<0.05)。结论 情景模拟演练能提高产科和手术室医护人员的工作效率,缩短DDI,有利于改善新生儿脐动脉血pH值。Objective To research the role of scenario simulation training in improving emergency cesarean section capacity and the impact on maternal and neonatal outcomes.Methods From January to December 2021, 84 obstetric and operation room medical workers were selected from the Third Affiliated Hospital of Guangzhou Medical University, then they were divided into scenario simulation training group and conventional training group according to random number table method, 42 medical workers in each group.The medical workers in conventional training group were provided conventional theory training, while the medical workers in scenario simulation training group were trained by scenario simulation training based on conventional training group.The two groups of medical staff were compared in the time from determining operation to entry, from entry to operation, decision to delivery interval(DDI), and neonatal and maternal outcomes.Results DDI, the time from determining operation to entry, and from entry to operation in scenario simulation training group were(9.34±4.89) minutes,(3.57±3.45) minutes, and(2.41±2.65) minutes, respectively;DDI, the time from determining operation to entry, and from entry to operation in conventional training group were(15.43±7.45) minutes,(7.32±5.67) minutes, and(5.78±3.21) minutes, respectively.There were statistically significant differences in DDI, the time from determining operation to entry, and from entry to operation between the two groups(t=4.621,2.373,4.892,P<0.05).There were statistically significant differences in operation time, bleeding volume of cesarean section, hospitalization time, and neontal umbilical artery pH value between the two groups(P<0.05).Conclusion Scenario simulation training can improve the work efficiency of obstetrical and operation room medical staff, and shorten DDI, which is beneficial to improve neonatal umbilical artery pH value.
