摘 要:目的探讨聚凝胺法、卡式微柱凝胶技术(简称“卡式法”)用于输血前筛查不规则抗体、鉴定血型的价值。方法选择685例输血患者,均于2019年6月-2023年8月期间在南昌市新建区中医医院接受检验,均接受聚凝胺法、卡式法检查,对比聚凝胺法、卡式法对不规则抗体检出情况,RhD、ABO血型鉴定准确性和检测时间。结果卡式法对ABO血型和RhD血型鉴定准确率均为99.71%(683/685),高于聚凝胺法的97.96%(671/685)、97.81%(670/685),有统计学差异(P<0.05);卡式法对不规则抗体总检出率为2.04%(14/685),高于聚凝胺法的0.29%(2/685),有统计学差异(P<0.05);卡式法检测时间为(5.12±1.248)min,短于聚凝胺法的(15.28±1.06)min,有统计学差异(P<0.05)。结论与聚凝胺法相比,使用卡式法可提高输血前不规则抗体检出率,准确鉴定血型,指导输血,具有较高的推广价值。Objective To explore the value of polyamine method and cartridge micro column gel technology(referred to as"cartridge method")in screening irregular antibodies and identifying blood types before blood transfusion.Method A total of 685 blood transfusion patients were selected,all of whom underwent testing at the Xinjian District Traditional Chinese Medicine Hospital in Nanchang City from June 2019 to August 2023.They all underwent tests using the polyamine method and the card method.The detection of irregular antibodies,accuracy of Rh D and ABO blood group identification,and testing time were compared between the polyamine method and the card method.Result The accuracy of ABO blood type and Rh D blood type identification using the card method was 99.71%(683/685),which was higher than the 97.96%(671/685)and 97.81%(670/685)of the polyamine method,with statistical differences(P<0.05);the total detection rate of irregular antibodies using the cartridge method was 2.04%(14/685),which was higher than the 0.29%(2/685)of the polyamine method,with a statistical difference(P<0.05);the detection time of the cartridge method was(5.12+1.248)minutes,which was shorter than the(15.28±1.06)minutes of the polyamine method,with a statistical difference(P<0.05).Conclusion Compared with the polyamine method,the card method can improve the detection rate of irregular antibodies before blood transfusion,accurately identify blood types,guide blood transfusion,and has high promotion value.
