肝硬化患者血小板参数、凝血功能指标与肝脏损害严重程度及上消化道出血的相关性研究

摘　　要：目的探讨血小板参数、凝血功能指标与肝硬化患者肝脏损害严重程度及上消化道出血的相关性。方法选取2021年1月-2024年1月天津市河东区大王庄街社区卫生服务中心诊疗的90例肝硬化患者,根据Chi1d-Pugh肝功分级将患者分为A、B、C级组,比较三组血小板参数、凝血功能指标差异。同时,将患者分为上消化道出血组及未出血组,分析两组血小板参数、凝血功能指标与上消化道出血相关性。结果肝硬化患者PDW、MPV、PT、APTT随肝损加重而升高(P<0.05),PLT、PCT、PTA、FIB随肝损加重而降低(P<0.05)。与未出血组比较,出血组PLT、PCT、PTA、FIB较低(P<0.05),PDW、MPV、PT、APTT较高(P<0.05)。PLT、PDW、MPV、PCT、PT、PTA、APTT、FIB临床意义与肝硬化上消化道出血呈正相关(P<0.05),且其诊断出血风险AUC分别为0.790、0.988、0.920、0.903、0.891、0.813、0.814、0.882(P<0.05)。结论血小板参数与凝血功能指标对于评估肝硬化患者肝脏损害严重程度及上消化道出血风险具有重要临床意义。Objective The paper explored correlation between platelet parameters,coagulation function indicators,liver damage and upper gastrointestinal bleeding in liver cirrhosis patients.Methods 90 patients with liver cirrhosis from January 2021 to January 2024 were first divided into group A,B and C,dfferences in platelet parameters and coagulation function indicators were compared,then they were divided into upper gastrointestinal bleeding group and non-bleeding group,correlation between platelet parameters,coagulation function indicators and upper gastrointestinal bleeding was analyzed.Results PDW,MPV,PT and APTT increased(P<O.05),while PLT,PCT,PTA and FIB decreased with aggravation of liver damage(P<0.05).Compared with non-bleeding group,bleeding group had lower PLT,PCT,PTA and FIB(P<0.05),higher PDW,MPV,PT and APTT(P<0.05).PLT,PDW,MPV,PCT,PT,PTA,APTT,FIB were positively correlated with upper gastrointestinal bleeding(P<0.05),diagnostic bleeding risk AUCs were 0.790,0.988,0.920,0.903,0.891,0.813,0.814 and 0.882,respectively(P<0.05).Conclusion For patients with liver crrhosis,platelet parameters and coagulation function indicators can help to evaluate severity of liver damage and risk of bleeding.
