摘 要:城市医疗集团是我国整合型医疗卫生服务体系的重要形式之一。南宁市整合各层级医疗机构资源, 由南宁市第一人民医院和第二人民医院这2家三级医院分别牵头组建了2个城市医疗集团, 并于2019年开始正式运行。由牵头医院的法定代表人兼任医疗集团中二级医院的法定代表人, 对二级医院的人、财、物和业务运营实行统一管理, 对基层医疗卫生机构则采用托管模式进行管理。通过建立集团内重大事项集体决策机制, 制定集团章程并成立集团办公室以推动集团内资源共享和同质化、一体化管理, 采取绩效分配激励机制鼓励专家下沉基层帮扶等措施, 有效促进了机构间的双向协作, 提升了成员单位的医疗服务水平。但仍存在体制机制不够完善、基层医疗卫生机构人才紧缺、学科建设能力不足、信息化建设相对滞后、牵头单位与成员单位的文化融合程度偏低等问题, 今后应通过完善顶层设计, 加大对基层医疗卫生机构的扶持力度, 优化学科建设布局, 提升信息化水平, 打造集团文化等进一步加强城市医疗集团建设。Urban medical group is one of the important forms of integrated medical and health service systems in China.Nanning City has integrated resources from various levels of medical institutions,and two urban medical groups have been established under the leadership of the tertiary hospitals of Nanning First People′s Hospital and Nanning Second People′s Hospital,respectively.These groups began to officially operate in 2019.The legal representative of the leading hospital also served as the legal representative of the secondary hospitals in the medical groups,implementing unified management of the personnel,finance,materials,and business operations of the secondary hospital,and adopting a custody mode for the management of grassroots medical and health institutions.By establishing a collective decision-making mechanism for major issues within the groups,formulating the group′s articles of association,and setting up group office to promote resource sharing,homogenization,and integrated management within the groups,and adopting performance allocation incentive mechanisms to encourage experts to go to grassroots medical and health institutions for assistance,the urban medical groups have effectively promoted two-way cooperation between institutions and improved the medical service level of member units.However,there were still problems such as incomplete institutional mechanisms,shortage of talents in grassroots medical and health institutions,insufficient disciplinary construction capacity,relatively lagging information technology construction,and low cultural integration between leading and member units.In the future,we should further strengthen the construction of urban medical groups by improving top-level design,increasing support for grassroots medical and health institutions,optimizing disciplinary construction layout,enhancing informatization level,and creating group culture.
