摘 要:目的:本文就藏药七十味珍珠丸(■)治疗缺血性脑卒中的研究现状及其药效物质基础等方面予以综述,为藏药七十味珍珠丸的临床应用提供参考。方法:本文通过在知网、PubMed等数据库检索“七十味珍珠丸”、“脑缺血”、“化学成分”、“毒理作用”等关键词,从抗脑缺血药效研究、化学成分研究、毒理研究等方面,对七十味珍珠丸的研究进展进行介绍。结果:藏药七十味珍珠丸对缺血性脑卒中疾病的药效学研究包括减小脑梗塞面积与抗血栓作用、改善脑血管通透性与抑制脑水肿形成、抗凋亡作用、抗氧化作用、神经保护作用、调节脑内代谢产物、镇静作用以及调节肠道菌群作用。相关药效物质基础研究主要是组方矿物药方面的研究,而组方中比例较大的植物药与动物药的研究相对较少。此外,七十味珍珠丸的毒理研究显示大鼠经口给予七十味珍珠丸的最大给药量为10g/kg(为成人日用剂量的600倍),安全性良好,但在临床应用时应监测血常规和血液生化学指标。结论:七十味珍珠丸抗缺血性中风的药效实验结果较确切,但相应的作用机制不够全面,药效物质基础研究也比较薄弱。Objective:This paper reviewed the current status of research on the treatment of ischemic stroke with the Tibetan medicine Qishiwei Zhenzhu pills(■)and its pharmacological and material bases,so as to provide reference for the clinical application of the Tibetan medicine Qishiwei Zhenzhu pills.Method:In this paper,by searching the keywords of"Qishiwei Zhenzhu pills","cerebral ischemia","chemical composition"and"toxicology"in the databases of Knowledge.com and PubMed,the research progress of Qishiwei Zhenzhu pills was summarized from the aspects of anti-ischemic efficacy research,chemical composition research and toxicology research.Result:Pharmacodynamic studies of Tibetan medicine Qishiwei Zhenzhu pills on ischemic stroke disease included reducing the area of cerebral infarction and antithrombotic effect,improving cerebrovascular permeability and inhibiting the formation of cerebral edema,anti-apoptotic effect,antioxidant effect,neuron-protective effect,regulating metabolites in the brain,sedative effect,as well as regulating the effect of intestinal flora.The research on the basis of related pharmacodynamic substances was mainly on the mineral medicine of the formula,while the research on the botanical medicine and animal medicine,which had a larger proportion in the formula,was relatively small.In addition,the toxicological study of Qishiwei Zhenzhu pills showed that the maximum dosage of Qishiwei Zhenzhu pills given orally to rats was 10 g·kg^(-1)(600 times of the daily dosage of adults),and the safety was good,but blood routine and blood biochemistry indexes should be monitored in the clinical application.Conclusion:The results of experiments on the eficacy of Qishiwei Zhenzhu pills against ischemic stroke are more accurate,but the corresponding mechanism of action is not comprehensive enough,and the basic research on the efficacy of the substance is also relativelyweak.
