清胰汤联合结肠途径治疗仪对重症急性胰腺炎患者胰腺功能及炎性因子的影响

摘　　要：目的探讨清胰汤联合结肠途径治疗仪对重症急性胰腺炎患者胰腺功能及炎性因子的影响。方法选择2021年1月至2022年12月医院住院治疗的116例重症急性胰腺炎患者,采用随机数字表法分为对照组和观察组,每组58例。对照组应用结肠途径治疗仪联合大黄浓煎液治疗,观察组应用结肠途径治疗仪联合清胰汤治疗。比较两组的临床效果、中医证候评分、乳酸水平及胰腺功能、炎性因子、不良反应。结果观察组总有效率高于对照组,差异有统计学意义(P<0.05);观察组中医证候评分低于对照组,差异有统计学意义(P<0.05);观察组乳酸水平低于对照组,差异有统计学意义(P<0.05);观察组胰岛素(INS)高于对照组,差异有统计学意义(P<0.05),葡萄糖(GLU)、血淀粉酶(AMY)、胰蛋白酶原-2(TPS2)低于对照组,差异有统计学意义(P<0.05);治疗后,观察组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)低于对照组,差异有统计学意义(P<0.05);两组不良反应比较,差异无统计学意义(P>0.05)。结论清胰汤联合结肠途径治疗仪可改善重症急性胰腺炎患者的胰腺功能,促使肠黏膜屏障功能恢复,减轻机体的炎性反应,改善临床症状。Objective To explore the effects of Qingyi Tang combined with colon approach therapy on pancreatic function and inflammatory factors in patients with severe acute pancreatitis.Methods A total of 116 patients with severe acute pancreatitis who were hospitalized in hospitals from January 2021 to December 2022 were selected and randomly divided into an observation group and a control group using a random number table method,with 58 cases in each group.The control group was treated with a colon pathway treatment instrument combined with rhubarb concentrated decoction,while the observation group was treated with a colon pathway treatment instrument combined with Qingyi decoction.The clinical effects,traditional Chinese medicine syndrome scores,lactate levels,pancreatic function,inflammatory factors,and adverse reactions of the two groups were compared.The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05);The TCM syndrome score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05);The lactate level in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05);The insulin(INS)in the observation group was higher than that in the control group,with a statistically significant difference(P<0.05).Glucose(GLU),blood amylase(AMY),and trypsinogen-2(TPS2)were lower than those in the control group,with a statistically significant difference(P<0.05);After treatment,the levels of interleukin-6(IL-6)and tumor necrosis factor(TNF-α)and interleukin-8 in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05);There was no statistically significant difference in adverse reactions between the two groups(P>0.05).Conclusion The combination of Qingyi Tang and colon pathway therapy device can improve pancreatic function in patients with severe acute panc
