摘　　要：慢性肾功能衰竭是临床常见病、多发病。中医治疗该病优势突出,何学红教授从事中医肾脏病临床40余年,对于疾病从中医病名、病机到治疗,形成完整的理论体系。认为其属郭恩绵教授提出的“虚劳水气病”范畴,并总结凝练“正虚毒蕴”的病机制论,认为脾肾亏虚为发病之根本,尤其重视从脾入手阐释肾脏病的病机,而其标实之证责之于毒邪内蕴,其既是病理产物,也是重要的致病因素,并认为毒邪贯穿于疾病始终,弥漫三焦,是迁延不愈,变证丛生的主要因素。在临床实践中,依据其基本病机,制定了“脾肾同治,排毒四法”的一体化治疗方案。方案以口服汤药参芪泄浊饮为核心以降浊毒;双白煎药浴以散浊毒;药物贴敷以祛浊毒;降氮煎剂保留灌肠疗法通腑以泄浊毒。内外合治的治疗方案,适用于慢性肾衰竭替代治疗前的各个时期,临床疗效显著。Chronic renal failure is a common clinical and frequently occurring disease.Traditional Chinese medicine has outstanding advantages on treating this disease.Professor HE Xuehong has been engaged in the clinic of kidney disease in traditional Chinese medicine for more than 40 years,and formed a complete theoretical system from the name,pathogenesis and treatment of the disease in traditional Chinese medicine.Thinking it belongs to the category of“deficiency of water and Qi disease”by Professor GUO Enmian,and summarizing the pathogenesis theory of“deficiency of positive deficiency and toxic accumulation”,and believing that deficiency of spleen and kidney is the root of the pathogenesis.She pays special attention to explain the pathogenesis of kidney disease from the perspective of spleen,and the symptoms of chronic renal failure are due to the pathogenic accumulation of toxic and pathogenic factors,which are pathological products and important pathogenic factors.And it is believed that toxins and evil spirits run through the entire disease,spreading throughout the three-Jiao,and are the main factors that persist and cause a cluster of symptoms.In clinical practice,according to its basic pathogenesis,the formulation of the“spleen and kidney treatment,detoxification of the four methods”integrated treatment program.In the scheme,the oral Shenqi Xiezhuo Yin(参芪泄浊饮)is taken as the core to reduce turbid poison;Shuangbai Decoction(双白煎)bath is to disperse turbid poison;Drugs applying is to remove turbidity and toxin;Jiangdan Decoction(降氮煎剂)retention enema therapy is to dredge intestinal tract to evacuate turbidity and toxin.The treatment scheme of internal and external treatment is suitable for all periods before the replacement treatment of chronic renal failure,and the clinical effect is remarkable.
