双切口三钢板内固定术治疗复杂胫骨平台骨折的临床效果

摘　　要：目的探讨复杂胫骨平台骨折(CTPF)患者采用双切口三钢板内固定术治疗的临床效果。方法选取2018年6月至2023年6月在范县人民医院接受治疗的60例CTPF患者,以随机数字表法分为研究组(n=30)和对照组(n=30),对照组实行单侧钢板内固定术,研究组实行双切口三钢板内固定术,对两组围手术期指标、膝关节恢复情况、复位效果、胫骨平台内翻角、胫骨平台内侧后倾角、并发症进行比较。结果研究组手术、骨折愈合、住院时间较对照组短,术中出血量较对照组少,负重时间较对照组早,差异具有统计学意义(P<0.05);术后6个月两组美国特种外科医院膝关节评分(HSS)较术前升高,且研究组HSS评分高于对照组,差异具有统计学意义(P<0.05);研究组术后复位优良率(100.00%)较对照组(80.00%)高,差异具有统计学意义(P<0.05);术后2个月两组胫骨平台内翻角、胫骨平台内侧后倾角较术前均有所增大,且研究组胫骨平台内翻角、胫骨平台内侧后倾角大于对照组,差异具有统计学意义(P<0.05);研究组术后并发症发生率(6.67%)较对照组(30.00%)低,差异具有统计学意义(P<0.05)。结论双切口三钢板内固定术应用于CTPF患者中,能够改善围手术期指标,促进关节复位及功能恢复,降低并发症发生风险。Objective To explore the clinical effect of using double-incision three-plate internal fixation surgery for the treatment of complex tibial plateau fractures(CTPF)in patients.Methods A total of sixty patients with CTPF who received treatment in Fan County People’s Hospital from June 2018 to June 2023 were randomly divided into study group(n=30)and control group(n=30)using a random number table method.The control group underwent unilateral steel plate internal fixation surgery,while the study group underwent double-incision three-plate internal fixation surgery.The perioperative indicators,knee joint recovery,reduction effect,tibial plateau inversion angle,tibial plateau medial posterior tilt angle,and complications of the two groups were compared.Results The study group had shorter surgery,fracture healing,and hospital stay compared to the control group,less intraoperative bleeding,and earlier weight-bearing time,with statistically significant differences(P<0.05);Six months after surgery,the knee joint score of American hospital for special surgery(HSS)of the two groups increased compared to before surgery,and the HSS score of the study group was higher than that of the control group,with statistical significance(P<0.05);The excellent rate of postoperative reduction in the study group(100.00%)was higher than that in the control group(80.00%),and the difference was statistically significant(P<0.05);Two months after surgery,the tibial plateau inversion angle and medial posterior inclination angle of the two groups increased compared to before surgery,and the tibial plateau inversion angle and medial posterior inclination angle of the study group were greater than those of the control group,with statistical significance(P<0.05);The incidence of postoperative complications in the study group(6.67%)was lower than that in the control group(30.00%),and the difference was statistically significant(P<0.05).Conclusion The application of double-incision three-plate internal fixation in CTPF patients can improve perioperat
