摘　　要：目的:探讨心脏远程实时监护系统在心血管疾病患者中的应用效果。方法:选取2022年9-12月于上海市第一人民医院嘉定医院使用心脏远程实时监护系统救治的207例心血管疾病患者作为研究对象,均进行常规12导联心电图(ECG)检查、12导联24 h动态心电图(Holter)检查、心脏远程监护仪(Holter 5000)检查,以临床综合诊断结果为“金标准”,比较三种检测方法心血管疾病检出情况以及对心血管疾病的诊断效能。结果:Holter 5000心肌缺血检出率高于ECG、 Holter,传导阻滞、心房扑动或心房颤动、室性早搏或室性心动过速检出率Holter 5000>Holter>ECG,Holter 5000、Holter房性早搏或房性心动过速高于ECG,差异有统计学意义(P<0.05);ECG、Holter心肌缺血检出率,Holter 5000、Holter房性早搏或房性心动过速检出率比较,差异无统计学意义(P>0.05)。Holter 5000诊断心血管疾病的灵敏度、特异度高于ECG、Holter,Holter诊断心血管疾病的特异度高于ECG,差异有统计学意义(P<0.05);ECG、Holter诊断心血管疾病的灵敏度比较,差异无统计学意义(P>0.05)。结论:心脏远程实时监护系统在心血管疾病患者中的应用效果显著,具有较高的灵敏度、特异度。Objective:To explore the application effect of cardiac remote real-time monitoring system in patients with cardiovascular disease.Methods:A total of 207 patients with cardiovascular disease treated by cardiac remote real-time monitoring system in Shanghai General Hospital Jiading Branch from September 2022 to December 2022 were selected as the study subjects.All patients underwent routine 12-lead electrocardiogram(ECG)examination,12-lead 24 h ambulatory electrocardiogram(Holter)examination and cardiac remote monitor(Holter 5000)examination.The clinical comprehensive diagnosis results were used as the"gold standard"to compare the detection of cardiovascular diseases by three detection methods and the diagnostic efficiency for cardiovascular diseases.Results:The detection rate of myocardial ischemia by Holter 5000 was higher than that by ECG and Holter;the detection rate of conduction block,atrial flutter or atrial fibrillation,premature ventricular contractions or ventricular tachycardia was ranked as follows:Holter 5000>Holter>ECG;the detection rate of premature atrial contractions or atrial tachycardia by Holter 5000 was higher than that by ECG,and the difference was statistically significant(P<0.05).There was no significant difference in the detection rate of myocardial ischemia between ECG and Holter,and in the detection rate of atrial premature contractions or atrial tachycardia between Holter 5000 and Holter(P>0.05).The sensitivity and specificity of Holter 5000 in the diagnosis of cardiovascular diseases were higher than those of ECG and Holter,and the specificity of Holter in the diagnosis of cardiovascular diseases was higher than that of ECG,and the difference was statistically significant(P<0.05).There was no significant difference in the sensitivity of ECG and Holter in diagnosing cardiovascular diseases(P>0.05).Conclusion:The application effect of cardiac remote real-time monitoring system in patients with cardiovascular diseases is remarkable,with high sensitivity and specificity.
