摘　　要：瘀热是国医大师周仲瑛教授于20世纪70年代率先提出的复合病理因素概念,现已逐步发展形成独特的瘀热辨证论治体系。本文总结导师陈四清教授运用周仲瑛“瘀热”理论,并结合“肝主疏泄、肝肾同源、肝主玄府”等理论治疗痤疮的临床经验。他提出各种原因引起的肝失疏泄是痤疮发生的中心环节,化热生瘀是痤疮难愈的重要原因,认为痤疮病位在肝,病机为肝火偏旺、湿热瘀郁,并以清肝泻火、凉血化瘀为治疗原则,应用于临床,获得良效。Stasis-heat is a concept of complex pathological factors first proposed in the 1970s by Professor ZHOU Zhongying,a master of traditional Chinese medicine.It has gradually developed into a unique system of syndrome differentiation and treatment of stasis-heat.This paper summarizes Professor CHEN Siqing's clinical experience in treating acne by using ZHOU Zhongying's theory of'stasis-heat',combined with the theory of'liver governing free flow of qi,liver and kidney from same source,liver governing sweat pore'.It is proposed that the liver failure in governing free flow of qi caused by various reasons is a central link of acne pathogenesis,and the heat and blood stasis are the important reasons for the difficulty of acne healing.It is believed that the acne disease is located in the liver,and the pathogenesis is liver fire,damp heat and blood stasis.The treatment principle is clearing liver and purging fire,cooling blood and resolving stasis,which has been applied in clinical practice and achieved good results.
