茯苓杏仁甘草汤联合呋塞米治疗慢性心力衰竭伴利尿剂抵抗临床观察

摘　　要：目的 观察茯苓杏仁甘草汤联合呋塞米治疗慢性心力衰竭伴利尿剂抵抗(CHFDR)临床疗效。方法 将我院90例CHFDR患者随机为治疗组与对照组各45例。对照组服用呋塞米治疗,治疗组采用茯苓杏仁甘草汤联合呋塞米治疗。比较两组总有效率、尿量、体质量指数、N端脑钠肽前体(NT-proBNP)、纽约心脏病学会心功能分级(NYHA)、左心室射血分数(LVEF)、中医主症和次症积分及住院时间、死亡率。结果 治疗后,治疗组总有效率高于对照组(P<0.05);两组尿量、LVEF高于治疗前,体质量指数、NT-proBNP、NYHA、中医主症和次症积分低于治疗前;治疗组尿量、LVEF高于对照组,体质量指数、NT-proBNP、NYHA、中医主症和次症积分低于对照组(P<0.05);治疗组住院时间少于对照组(P<0.05);两组死亡率差异无统计学意义(P>0.05)。结论 茯苓杏仁甘草汤联合呋塞米可提高CHFDR患者临床疗效,显著利尿,提高心功能,降低中医主症和次症积分,缩短住院时间,但对死亡率无明显影响。Objective To observe the clinical efficacy of Fuling Xingren Gancao Decoction combined with furosemide in treatment of chronic heart failure with diuretic resistance(CHFDR).Methods Ninety patients with CHFDR were randomly divided into treatment group and control group,with 45 cases in each group.Control group was treated with furosemide,while treatment group was treated with uling Xingren Gancao Decoction on the basis of control group.The total effective rate,urine output,body mass index,N-terminal brain natriuretic peptide precursor(NT-proBNP),New York College of Cardiology cardiac function classification(NYHA),left ventricular ejection fraction(LVEF),TCM main and secondary disease scores,length of hospital stay and mortality were compared between the two groups.Results After treatment,the total effective rate of treatment group was higher than control group(P<0.05),and the urine output and LVEF of both groups were higher than before treatment,which were higher in treatment group than control group,while the body mass index,NT-proBNP,NYHA,and the scores of TCM main and secondary symptoms in both groups were lower than before treatment,which were lower in treatment group than control group(P<0.05);the length of hospital stay in treatment group was lower than control group(P<0.05),and there was no significant difference in mortality between the two groups(P>0.05).Conclusion Fuling Xingren Gancao Decoction combined with furosemide can improve the clinical efficacy,significantly diuretic,improve cardiac function,reduce the score of TCM major and secondary symptoms,and shorten the length of hospital stay,but has no significant effect on mortality.
