摘 要:目的了解甘肃省职业人员外照射个人剂量水平,为放射人员健康管理和放射性疾病预防提供参考。方法依照国家标准对甘肃省2021年放射工作人员开展个人剂量监测和评价,使用RE2000A全自动热释光剂量读出仪和精密退火炉进行测读和退火,使用SPSS 22.0进行统计分析。结果本次分析包括273家单位的4031名放射工作人员,受监测人员的人均年有效剂量为0.28 mSv/a。不同职业类别有效剂量分布差异有统计学意义(H=147.22,P<0.05),教育、动物医院等其他单位有效剂量(0.30 mSv/a)高于医学应用类人员(0.28 mSv/a),差异有统计学意义(Z=-10.82,P<0.05)。医学应用类别中核医学的有效剂量最高(0.47 mSv/a),其次为介入放射学(0.31 mSv/a);不同级别医院的监测结果分布差异有统计学意义(H=20.69,P<0.05),三级医院的人均年有效剂量最低(0.25 mSv/a)。结论甘肃省2021年职业性外照射个人剂量总体较低,应继续提高放射工作人员的重视度,避免佩戴错误、故意照射等情况的发生。加强工业应用单位,教育、科研及动物医院等机构的防护和监管,医疗机构应加强介入和核医学人员的局部器官和内照射监测。Objective To analyze the personal dose level of occupational external exposure of radiation workers in Gansu Province,provide references for health management of radiation personnel and prevention of radiation diseases.Methods The personal dose monitoring and evaluation of radiation workers in Gansu Province in 2021 was conducted according to national standards.The RE2000A fully automatic thermoluminescence dose readout instrument and precision annealing furnace was applied for the measurement and annealing.The statistical analysis was performed using SPSS 22.0.Results A total of 4031 radiation workers from 273 units were collected in this analysis.The annual effective dose of monitored person was 0.28 mSv/a.There was statistically significant differences in effective dose distribution among different occupational categories(H=147.22,P<0.05).The effective dose of other units such as education and animal hospitals(0.30 mSv/a)was higher than that of medical application personnel(0.28 mSv/a),and the difference was statistically significant(Z=-10.82,P<0.05).Among the medical application categories,nuclear medicine had the highest effective dose(0.47 mSv/a),followed by interventional radiology(0.31 mSv/a).There was a statistically significant difference in the distribution of monitoring results among different levels of hospitals(H=20.69,P<0.05),and the annual effective dose of individual monitoring results in tertiary hospital was lowest(0.25 mSv/a).Conclusions The personal dose of occupational external exposure in Gansu Province is generally low in 2021.It is necessary to continue to increase the importance of radiation workers and avoid wearing errors,intentional exposure,and other situations.The protection and supervision of industrial application units,education,scientific research,and animal hospitals and other institutions should be strengthened.The medical institutions should strengthen monitoring of local organs and internal radiation for intervention and nuclear medicine personnel.
