基于聚类分析与关联规则方法探讨补肾益肺法治疗肺纤维化用药规律

摘　　要：目的 基于中医药治疗肺纤维化(pulmonary fibrosis,PF)的临床随机对照试验,分析现代中医专家治疗PF的用药规律。方法 本研究利用国内数据库万方数据库,中国知网以及维普网进行文献检索。根据纳入及排除标准筛选文献,从中提取并录入组方,建立数据库及规范药物名称,按照药物分类、药味、药性、归经进行频次统计。结合中医理论,对高频药物进行聚类分析、关联规则分析,总结用药规律。结果 出现频次最多的药物是甘草、黄芪、五味子、当归、熟地黄;出现频次最多的药物类别补虚药、活血化瘀药、化痰止咳平喘药、收涩药。药性以温性为主,药味多见甘、苦、辛。聚类分析提取出5个聚集群;关联规则分析得出药物关联规则46条。结论 现代临床专家基于PF肺肾两虚为本,痰瘀互结的疾病本质,在活血化瘀、祛痰平喘的同时,着重补益肺肾,标本兼顾,扶正祛邪,发挥中医药在治疗PF诸类难治疾病上的优势。Objective Based on the randomized controlled trials of Chinese medicine in the treatment of PF,to analyze the medication rules of modern Chinese medicine experts.Methods We searched CNKI,VIP,Wanfang Data.Articles were screened according to inclusion and exclusion criteria,and the prescriptions were extracted and entered.The database were established and the drug names were standardized,while the frequency characteristics were carried out according to the drug classification,taste,property and meridian tropism.Combined with the theory of Chinese medicine,cluster analysis and association rule analysis of high-frequency drugs were carried out to summarize the medication rules.Results Licorice,Astragalus membranaceus,Schisandra chinensis,Angelica sinensis and Rehmannia glutinosa were the most frequently used drugs;The most frequent drug categories were Tonic drugs,Activating Blood Circulation drugs,drugs for reducing phlegm and relieving cough and antiasthmatic,and Astringent drugs.The property was mainly warm,and the tastes was sweet,bitter and acrid..Five clusters were extracted by cluster analysis and 46 drug association rules by association rule analysis.Conclusion Clinical experts,based on the pathogenesis of PF disease of kidney-lung deficiency and phlegm-blood stasis,focus on nourishing lung and kidney,addressing both symptoms and root causes,strengthening healthy qi to elimin ate pathogens,and giving full play to the advantages of medicine in treating such incurablediseasesas PE.
