退变性脊柱侧弯经后路腰椎椎间植骨融合椎弓根钉棒内固定术对腰椎功能及Cobb角和疼痛程度的影响

摘　　要：目的 探讨退变性脊柱侧弯(degenerative scoliosis, DS)采用经后路腰椎椎间植骨融合内固定手术对腰椎功能、Cobb角及疼痛程度的影响。方法 选取2021年4月—2022年10月郑州大学第一附属医院收治的DS患者49例,患者均接受经后路腰椎椎间植骨融合椎弓根钉棒内固定术治疗。比较术前和术后6个月患者腰椎功能、Cobb角及疼痛程度。结果 术后6个月经日本骨科协会评估治疗分数(Japanese Orthopaedic Association scores, JOA)评分(21.19±1.43)分,比手术前显著增高(P<0.05),术后Oswestry功能障碍指数(Oswestry Disability Index, ODI)评分(20.51±2.06)分,比手术前显著降低(P<0.05);术后6个月患者侧凸Cobb角及后突Cobb角分别为(16.45±4.57)°及(18.36±4.57)°,比手术前显著减少(P<0.05);手术前患者VAS评分为(7.78±1.25)分,术后6个月VAS评分为(2.01±0.31)分,与术前比较有统计学意义(P<0.05)。结论 将腰椎椎间植骨融合固定术应用于退变性脊柱侧弯患者治疗中,可有效改善患者腰椎功能,降低疼痛感,改善患者的日常生活质量,提高患者满意度,值得推广。Objective To observe and analyze the therapeutic effect of lumbar interbody bone grafting(posterior approach) fusion internal fixation(pedicle screw rod) in patients with scoliosis(degenerative).Methods A total of 49 patients with DS admitted to the First Affiliated Hospital of Zhengzhou University from April 2021 to October 2022 were selected. All of the patients underwent posterior lumbar interbody bone graft fusion with pedicle nail rod internal fixation. Lumbar function, Cobb Angle and pain degree were compared before and 6 months after surgery.Results The score of Japanese Orthopaedic Association scores(JOA) was(21.19±1.43) points 6 months after surgery, which was significantly higher than that before surgery(P<0.05). The Oswestry Disability Index(ODI) score(20.51±2.06) was significantly lower than that before operation(P<0.05). The Cobb Angle of scoliosis and posterior process were(16.45±4.57) ° and(18.36±4.57) °, respectively, 6 months after operation, which were significantly reduced than before operation(P<0.05). The VAS score of the patients was(7.78±1.25) points before operation and(2.01±0.31) points 6 months after operation, which had statistical significance compared with that before operation(P<0.05).Conclusion The application of lumbar interbody bone graft fusion fixation in the treatment of patients with degenerative scoliosis can effectively improve the function of the lumbar spine, reduce pain, improve the quality of daily life of patients, and increase patient satisfaction, which is worthy of promotion.
