经筋推拿联合穴位熏蒸治疗肩周炎(寒湿痹阻证)临床观察

摘　　要：目的观察经筋推拿结合穴位熏蒸治疗肩周炎(寒湿痹阻证)的临床疗效。方法将60例患者用随机数字表法分为对照组(常规推拿结合穴位熏蒸)与观察组(对照组基础上加经筋手法)各30例,对比两组诊疗前后的视觉模拟疼痛量表(VAS)评分,前屈、后伸、内旋、外展4个维度的肩关节活动角度(ROM)变化以及两组临床疗效。结果观察组临床总有效率为90.00%,显著高于对照组的83.33%(P<0.05);两组治疗后VAS评分比较,观察组显著低于对照组(P<0.05);两组治疗后ROM比较,观察组的前屈、后伸、内旋、外展的数值均大于对照组(P<0.05)。结论经筋推拿结合穴位熏蒸治疗肩周炎(寒湿痹阻证)临床疗效确切,可以有效缓解疼痛,显著扩大肩关节的活动角度和改善肩部功能状态。Objective:To observe the clinical efficacy of massage through tendons combined with acupoint fumi⁃gation in the treatment of shoulder periarthritis(syndrome of cold-damp blockade).Methods:A total of 60 pa⁃tients were randomly divided into the control group(30 cases receiving conventional massage combined with acu⁃point fumigation)and observation group(30 cases receiving massage through tendons on the basis of the control group).Visual analogue pain scale(VAS)scores of the two groups before and after treatment were compared,and the outcome indicators such as the changes of shoulder joint motion angle(ROM)in four dimensions of forward flexion,posterior extension,internal rotation and abduction before and after treatment were compared.The clinical effects between two groups were also compared.Results:After treatment,the total clinical effective rate of the ob⁃servation group was 90.00%,which was significantly higher than 83.33%of the control group(P<0.05);the VAS score of the observation group was significantly lower than that of the control group(P<0.05),and the values of forward flexion,posterior extension,internal rotation and abduction in the observation group were higher than those in the control group(P<0.05).Conclusion:The clinical efficacy of massage through tendons combined with acu⁃point fumigation in the treatment of shoulder periarthritis(syndrome of cold-damp blockade)is reliable,which can effectively alleviate the pain of patients,significantly expand the angle of motion of the shoulder joint and im⁃prove the functional status of the shoulder.
