摘　　要：目的:分析肺结核患者的疾病负担现状和影响因素,为完善肺结核患者保障政策提供参考依据。方法:采用自行设计的问卷,对肺结核患者开展调查,收集其社会人口学特征、治疗情况、疾病负担情况等,采用多因素Logistic回归分析肺结核患者发生灾难性卫生支出的影响因素。结果:耐药肺结核患者的疾病经济负担远高于普通肺结核患者,54.46%的患者发生了灾难性卫生支出,外地户籍、耐药肺结核、有合并症、对肺结核知识了解程度一般,是发生灾难性卫生支出的危险因素,更高的家庭年收入是保护因素。结论:肺结核患者尤其是耐药肺结核患者面临着沉重的疾病经济负担,亟需完善肺结核保障政策,除重视经济负担外,还需关注肺结核患者的焦虑、痛苦等心理负担,加强对肺结核患者的综合支持。Objective To analyze the current situation and influencing factors of the disease burden in patients with pulmonary tuberculosis,and to provide a reference for improving the protection policy for pulmonary tuberculosis patients.Methods A self-designed questionnaire was used to gather information such as socioeconomic characteristics,treatment status,disease burden and other relevant factors from pulmonary tuberculosis patients.Multifactorial logistic regression was used to analyze the influencing factors contributing to catastrophic health expenditures.Results The economic burden of disease in drug-resistant tuberculosis patients was much higher than that of ordinary tuberculosis patients.54.46%of patients had catastrophic health expenditures.Non-resident registration,drug-resistant tuberculosis,comorbidities,and general knowledge of tuberculosis were risk factors for catastrophic health expenditures,while higher annual household income was a protective factor for catastrophic health expenditures.Conclusion Patients with pulmonary tuberculosis,especially those with drug-resistant tuberculosis,face a heavy economic burden of disease,and there is an urgent need to improve protection policy for tuberculosis patients.In addition to focusing on the economic burden,it is also necessary to pay attention to the psychological burdens of anxiety and pain of patients with tuberculosis,and strengthen the comprehensive support for patients with tuberculosis.
