摘　　要：目的探讨不同分娩方式对初产妇母婴结局、产后盆底功能的影响。方法选取青岛市胶州中心医院2020年1月至2021年12月收治的324例初产妇作为研究对象,按照随机数字表法分为观察组(采用会阴无保护分娩方式)162例与对照组(采用会阴保护分娩方式)162例。观察两组的会阴情况、生产情况、母婴结局以及盆底功能。结果观察组会阴裂度为0度与Ⅰ度的例数以及会阴无裂伤比例明显高于对照组(P<0.05)。观察组初产妇疼痛评分、出血量均低于对照组初产妇(P<0.05)。观察组新生儿窒息、宫内感染、产妇产后出血与软产道撕裂等发生率明显低于对照组(P<0.05),初产妇产后出血与软产道撕裂等发生情况低于对照组(P<0.05)。结论初产妇会阴无保护分娩方式有一定改善母婴结局与产后盆底功能的作用。Objective To explore the influence of different delivery methods on the maternal-infant outcomes and postpartum pelvic floor function in primiparae.Methods A total of 324 primiparae admitted to Jiaocheng Central Hospital of Qingdao from January 2020 to December 2021 were selected as the research subjects.They were divided into the observation group(162 cases,using unprotected perineal delivery)and the control group(162 cases,using protected perineal delivery)by using the random number table method.The perineal condition,delivey status,maternal-infant outcomes,and pelvic floor function were observed in both groups.Results The number of cases with 0 degree and the number of cases withⅠdegree of perineal laceration in the observation group were significantly higher compared to the control group(P<0.05).The pain score and blood loss of primiparae in the observation group were lower than those in the control group(P<0.05).The incidence rates of neonatal asphyxia,intrauterine infection,postpartum hemorrhage and soft birth canal tear in the observation group were significantly lower than those in the control group(P<0.05).Conclusion The use of unprotected perineal delivery in primiparae has a certain effect on improving maternal-infant outcomes and postpartum pelvic floor function.
