重度子痫前期患者妊娠晚期血清BNP及左室功能变化分析

摘　　要：目的探究重度子痫前期(sPE)患者妊娠晚期血清脑钠肽(BNP)及左室功能变化特征。方法回顾性分析2021年1月至2023年1月成都市锦江区妇幼保健院收治的112例sPE患者的临床及超声资料。患者根据发病时间分为早发型组(43例)与晚发型组(69例)。比较分析两组的血清BNP及左室结构、功能参数指标。结果早发型组的血清BNP显著高于晚发型组(P<0.05)。早发型组的E/A、e’/s’显著低于晚发型组,而IVRT和LAVI显著高于晚发型组(P<0.05),其余指标组间差异均无统计学意义(P>0.05)。Pearson相关性分析显示,血清BNP水平与E/A、e’/s’呈负相关性(r=-0.381、-0.273,P<0.05),与LVAI、IVRT呈显著正相关性(r=0.271、0.258,P<0.05)。结论早发型sPE患者的左室舒张功能减低较晚发型有加重趋势,BNP升高在一定程度上反映患者的左室舒张功能减低。Objective To investigate the changes of serum brain natriuretic peptide(BNP)and left ventricular function in late pregnancy of pregnant women with severe preeclampsia(sPE).Methods A retrospective analysis was conducted on the clinical and ultrasound data of 112 sPE patients admitted to the Jinjiang Maternity and Child Health Hospital of Chengdu from January 2021 to January 2023.Patients were divided into early onset group(43 cases)and late onset group(69 cases)based on the onset time.The serum BNP and left ventricular structure and functional parameters.Results The serum BNP in the early onset group was significantly higher than that in the late onset group(P<0.05).The E/A and e'/s'of the early onset group were significantly lower than those of the late onset group,while IVRT and LAVI were significantly higher than those of the late onset group(P<0.05),but there was no statistically significant difference in other indicators between the two groups(P>0.05).Pearson correlation analysis showed that serum BNP level was negatively correlated with E/A and e'/s'(r=-0.381,-0.273,P<0.05),and significantly positively correlated with LVAI and IVRT(r=0.271,0.258,P<0.05).Conclusion Compared with late onset sPE patients,left ventricular diastolic function is inclined to decrease in early onset sPE patients,and the increase of serum BNP levels can reflect the patient's left ventricular diastolic function reduction to some extent.
