摘　　要：目的 了解湖北省荆州市居民健康水平,测算不同性别和地区成人健康期望寿命,为卫生决策提供依据。方法采用多阶段分层随机抽样方法随机抽取荆州市常住居民,利用WHO自评健康调查问卷,开展自评健康状况调查。基于hierarchial ordered probit(HOPIT)模型矫正自报健康数据得到伤残测度,用沙利文法计算居民健康期望寿命。结果荆州市18岁居民健康期望寿命(HALE)为36.16岁,男性35.96岁,女性36.73岁;城市人群36.85岁,农村人群35.63岁,随着年龄增长,居民伤残损失寿命占期望寿命比例(LHE/LE)不断上升。结论荆州市居民健康期望寿命存在性别和城乡差异;应加强老年人群综合健康管理,延长健康寿命。Objective To understand the healthy level of residents in Jingzhou city of Hubei province,calculate the healthy life expectancy of adults in different genders and regions,and provide basis for developing health policies.Methods The permanent residents were randomly selected with multi-stage stratified random sampling method in Jingzhou,and the self-rated health status survey was carried out with the WHO self-rated health questionnaire.Based on hierarchial ordered probit(HOPIT)model,the disability measures was obtained through rectify self-reported health data,and the healthy life expectancy of the residents was calculated with Sullivan method.Results The healthy life expectancy(HALE)of 18 years old was 36.16 years old in the residents of Jingzhou city,of which,the HALE of the male was 35.96 years old and that of the female was 36.73 years old.The HALE of the urban population was 36.85 years old and that of the rural population was 35.63 years old.The proportion of life lost due to disability to life expectancy(LHE/LE)kept increasing with the age.Conclusions There are statistical differences in the HALEs of the residents between genders and between urban and rural population in Jingzhou city.The comprehensive health management of elder population should be strengthened for prolonging the healthspan.
