摘　　要：脊髓型颈椎病是一个统称,多指由于颈椎管狭窄、与年龄相关变化(后方骨赘、椎间盘压缩和后突出),以及颈椎不稳定或轻度创伤等一系列导致脊髓受压的疾病,严重时可致瘫痪。一般来说,颈髓受压静态因素包括狭窄和骨刺,动态因素包括不稳定和创伤,而动、静态因素相互影响又常同时存在的性质使多节段脊髓受压往往更为常见。颈椎管扩大成型术(expansive open-door laminoplasty,EOLP)广泛应用于多节段脊髓型颈椎病的手术治疗中,能够通过释放椎管容积而获得满意疗效。因其手术范围选择对各种术后并发症有一定影响,是医患双方对预后状态评估疗效主要困扰所在。因此本文就近年EOLP手术范围对轴性症状、C5神经根麻痹、颈椎后凸畸形、颈椎活动度丧失等术后常见并发症的相关报道进行综述。Spondylotic cervical spondylosis is an umbrella term that mostly refers to a series of disorders that result in spinal cord compression due to cervical spinal stenosis,age-related changes(posterior osteophytes,disc compression and poste-rior herniation),as well as cervical spinal instability or mild trauma that can lead to paralysis in severe cases.In general,static factors for cervical cord compression include stenosis and bone spurs,dynamic factors include instability and trauma,and the nature of the dynamic and static factors interacting and often co-existing makes multisegmental spinal cord compression often more common.Expansive open-door laminoplasty(EOLP)is widely used in the surgical treatment of multisegmental cervical spondylolisthesis,which can achieve satisfactory results by releasing the volume of the spinal canal.Because the choice of sur-gical scope has a certain impact on various postoperative complications,it is a major problem for both doctors and patients to as-sess the prognosis of the treatment.Therefore,in this article,we review the recent reports of common postoperative complica-tions such as axial symptoms,C5 nerve root palsy,cervical kyphosis,and loss of cervical spine mobility associated with the sur-gical scope of EOLP.
