黄芪桂枝五物汤治疗化疗后周围神经毒性

摘　　要：目的探讨黄芪桂枝五物汤熏洗治疗化疗后周围神经毒性(OIPN)的疗效。方法采用随机数表法将108例化疗后周围神经毒性肿瘤患者分为对照组与观察组,各54例。对照组给予甲钴胺片治疗,观察组给予黄芪桂枝五物汤加减熏洗。均持续治疗2个月。比较2组临床疗效、周围神经传导速度、周围神经毒性分级、炎性因子、肿瘤标志物及不良反应发生情况。结果观察组治疗2个月后的总有效率(96.30%,52/54)高于对照组(59.26%,32/54)(P<0.05)。治疗2个月后;2组周围神经传导速度均快于治疗前,观察组快于对照组(P<0.05);2组周围神经毒性0级的患者占比均高于治疗前,观察组高于对照组(P<0.05);2组1级及观察组2级、3级的患者占比均比治疗前低,观察组较低;2组炎性因子及肿瘤标志物水平均比治疗前低,且观察组血清炎性因子水平较低(P<0.05)。2组治疗期间的不良反应发生率比较,差异无统计学意义(P>0.05)。结论黄芪桂枝五物汤熏洗有助于提高化疗后周围神经毒性肿瘤患者周围神经传导速度,降低周围神经毒性严重程度,并有效减轻患者炎症反应,提高治疗效果,且安全性良好。Objective To explore the therapeutic effect of the fumigation and soaking therapy with Huangqi Guizhi Wuwu decoction in the treatment of oxaliplatin-induced peripheral neurotoxicity(OIPN)after chemotherapy.Methods A total of 108 tumor patients with OIPN after chemotherapy were divided into a control group and an observation group by the random number table method,with 54 cases in each group.The control group was treated with mecobalamin tablets,while the observation group was treated with the fumigation and soaking therapy with modified Huangqi Guizhi Wuwu decoction.Both groups were treated continuously for two months.The clinical efficacy,peripheral nerve conduction velocity,OIPN grading,levels of inflammatory factors and tumor markers,as well as the incidence of adverse reactions were compared between two groups.Results The total effective rate of the observation group(96.30%,52/54)was higher than that of the control group(59.26%,32/54)after 2 months of treatment(P<0.05).After 2 months of treatment,the peripheral nerve conduction velocity was faster than before treatment in both groups,while the observation group was faster than the control group(P<0.05).The proportion of patients with OPIN grade 0 was higher than before treatment in both groups,and the observation group was higher than the control group(P<0.05).The proportion of patients with grade 1 in both groups and those with grade 2 and grade 3 in the observation group was lower than before treatment,and the observation group was much lower(P<0.05).The levels of inflammatory factors and tumor markers in both groups were lower than before treatment,and the levels of inflammatory factors in the observation group were much lower(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups during the treatment period(P>0.05).Conclusion In the treatment of tumor patients with oxaliplatin-induced peripheral neurotoxicity after chemotherapy,the fumigation and soaking therapy with Huangqi Guizhi Wuwu decoc
