补肾调周法治疗心因性黄体功能不全性不孕症的临床研究

摘　　要：目的分析补肾调周法治疗心因性黄体功能不全性不孕症的疗效。方法选取本院40例心因性黄体功能不全性不孕症患者为研究对象,采用队列研究的方法,将其分为两组,各20例。中药组患者采用补肾调周法序贯治疗,西药组患者采用来曲唑促排治疗。观察两组治疗前后生活事件量表积分、临床妊娠率、基础状态下黄体生成素(LH)、黄体中期孕酮(P)和雌二醇(E_(2))、亲吻素、促肾上腺皮质激素、皮质醇水平变化情况及中医证候积分。结果治疗后,两组基础状态下LH及黄体中期E_(2)、P水平、血浆亲吻素值均明显升高,且中药组高于西药组(P<0.05)。两组促肾上腺皮质激素水平、皮质醇水平、中医证候积分及生活事件量表评分均降低,且中药组低于西药组(P<0.05)。中药组临床妊娠率为40%,高于西药组的20%(P<0.05)。中药组治疗总有效率显著高于西药组(P<0.05)。治疗期间,两组患者均未见严重不良反应。结论采用补肾调周法治疗心因性黄体功能不全性不孕症疗效确切,可显著改善患者临床症状,并能有效提高患者的妊娠率,用药安全。Objective To analyze the therapeutic effect of tonifying the kidney and regulating the menstrual cycle method on psychogenic luteal insufficiency infertility.Methods40 patients with psychogenic luteal insufficiency infertility in our hospital were selected as the research subjects,and a cohort study was used to divide them into two groups,with 20 cases in each group.Patients in the traditional Chinese medicine group were treated with sequential therapy of tonifying the kidney and regulating the menstrual cycle,while patients in the western medicine group were treated with letrozole to promote ovulation.Observe the changes in life event scale scores,clinical pregnancy rates,basal luteinizing hormone(LH),mid luteal progesterone(P)and estradiol(E_(2)),kissing hormone,adrenocorticotropic hormone,cortisol levels,and traditional Chinese medicine syndrome scores before and after treatment in two groups.Results After treatment,the levels of LH and mid luteal E_(2) and P,as well as plasma kissing hormone levels,were significantly increased in both groups at baseline,and the traditional Chinese medicine group was higher than the western medicine group(P<0.05).The levels of adrenocorticotropic hormone,cortisol,traditional Chinese medicine syndrome scores,and life event scale scores were all reduced in both groups,and the traditional Chinese medicine group was lower than the western medicine group(P<0.05).The clinical pregnancy rate in the traditional Chinese medicine group was 40%,which was higher than 20%in the Western medicine group(P<0.05).The total effective rate of the traditional Chinese medicine group was significantly higher than that of the western medicine group(P<0.05).During the treatment period,no serious adverse reactions were observed in both groups of patients.Conclusion The treatment of psychogenic luteal insufficiency infertility using the method of tonifying the kidney and regulating the menstrual cycle has a definite therapeutic effect,can significantly improve the clinical symptoms of patients,and can e
