腰-硬联合麻醉与持续硬膜外麻醉用于剖宫产手术的临床效果比较

摘　　要：目的比较腰-硬联合麻醉与持续硬膜外麻醉用于剖宫产手术的临床效果。方法选取330例剖宫产孕妇,根据随机数字表法分为研究组与对照组,各165例。对照组采用持续硬膜外麻醉,研究组采用腰-硬联合麻醉。比较两组麻醉效果。结果研究组麻醉起效时间、完全阻滞时间、剖宫产时间均短于对照组,麻醉药物用量、手术中输液量均少于对照组,手术中平均动脉压(MAP)水平高于对照组,手术中心率(HR)水平低于对照组(P<0.05)。两组新生儿出生5min的Apgar评分比较差异无统计学意义(P>0.05)。研究组并发症发生率4.24%低于对照组的12.73%(P<0.05)。结论剖宫产孕妇予以腰-硬联合麻醉,可作为剖宫产手术中的首选麻醉方式。Objective To compare the clinical effects of combined lumbo-epidural anesthesia and continuous epidural anesthesia for cesarean section,Methods 330 pregnant women with cesarean section were selected and divided into study group and control group according to random number table method,165 cases in each group.The control group received continuous epidural anesthesia,and the study group received lumbo-epidural anesthesia.The anesthetic effect of the two groups was compared.Results The onset time of anesthesia,complete block time and cesarean section time of the study group were shorter than those of the control group,the dosage of anesthesia drugs and the amount of intraoperative infusion were lower than those of the control group,the level of intraoperative mean arterial pressure(MAP)was higher than that of the control group,and the level of surgical center rate(HR)was lower than that of the control group(P<0.05).There was no significant difference in Apgar score at 5 min after birth between the two groups(P>0.05).The complication rate of 4.24% in the study group was lower than 12.73% in the control group(P<0.05).Conclusion Lumbo-epidural anesthesia should be given to pregnant women during cesarean section,which can be used as the preferred anesthesia method in cesarean section operation.
