小剂量硬膜外麻醉复合浅全麻在宫外孕合并休克手术中的应用效果

摘　　要：目的探讨宫外孕合并休克患者术中采用小剂量硬膜外麻醉复合浅全身麻醉的临床效果。方法选取100例采用手术治疗宫外孕合并休克的患者,依照术中麻醉方式不同将其分为两组,每组50例。对照组患者术中采用气管插管浅全身麻醉,观察组患者术中采用小剂量硬膜外麻醉复合浅全身麻醉,对比两组不同时间点的生命体征、手术时间、苏醒时间和拔管时间。结果观察组在麻醉开始时(T_(1))、手术开始时(T_(2))、手术结束时(T_(3))的心率低于对照组,平均动脉压高于对照组(P<0.05)。观察组的手术时间、苏醒时间、拔管时间均短于对照组(P<0.05)。结论宫外孕合并休克患者在术中采用小剂量硬膜外麻醉复合浅全身麻醉的效果较理想,不仅可以有效稳定其生命体征,还可以缩短手术时间和术后恢复时间。Objective To investigate the clinical effect of low dose epidural anesthesia combined with shallow general anesthesia in patients with ectopic pregnancy combined with shock.Methods 100 patients with ectopic pregnancy combined with shock were selected and divided into two groups according to different anesthesia methods,with 50 cases in each group.Patients in the control group were given superficial general anesthesia with tracheal intubation during the operation,and patients in the observation group were given low-dose epidural anesthesia combined with shallow general anesthesia during the operation.The vital signs,operation time,recovery time and extubation time of the two groups were compared at different time points.Results The heart rate at the beginning of anesthesia(T_(1)),the beginning of surgery(T_(2))and the end of surgery(T_(3))in the observation group was lower than that in the control group,and the mean arterial pressure was higher than that in the control group(P<0.05).The operation time,recovery time and extubation time of observation group were shorter than those of control group(P<0.05).Conclusion The effect of low dose epidural anesthesia combined with shallow general anesthesia is ideal in patients with ectopic pregnancy combined with shock,which can not only stabilize their vital signs effectively,but also shorten the operation time and postoperative recovery time.
