左侧倾斜卧位对自然分娩产妇分娩进程及母婴结局的影响

摘　　要：目的观察左侧倾斜卧位对自然分娩产妇分娩进程及母婴结局的影响。方法选取94例自然分娩的产妇,根据随机数字表法分为观察组与对照组,各47例。对照组实施仰卧位分娩,观察组实施左侧倾斜卧位分娩。比较两组分娩进程、母婴结局、分娩舒适度。结果观察组剖宫产率、会阴撕裂率、胎儿窘迫率分别为10.64%、19.15%、2.13%,均低于对照组的27.66%、38.30%、14.89%,自然分娩率89.36%高于对照组的72.34%(P<0.05)。两组产后出血率、新生儿窒息率比较差异无统计学意义(P>0.05)。观察组分娩总舒适度为91.49%,高于对照组的74.47%(P<0.05)。结论自然分娩产妇以左侧倾斜卧位进行分娩,能够有效加快分娩进程,降低剖宫产率、会阴撕裂率、胎儿窘迫率,获得良好的分娩舒适度,值得临床推广。Objective To observe the effects of left oblique decubitus position on the process of labor and maternal and infant outcomes of natural parturients.Methods 94 natural parturients were selected and divided into observation group and control group according to random number table method,47 cases in each group.The control group delivered in supine position,and the observation group delivered in left oblique position.The labor process,maternal and infant outcomes,and labor comfort were compared between the two groups.Results The cesarean section rate,perineal tear rate and fetal distress rate in the observation group were 10.64%,19.15% and 2.13%,all lower than 27.66%,38.30% and 14.89% in the control group,and the natural delivery rate was 89.36%,higher than 72.34%(P<0.05).There was no significant difference in the rate of postpartum hemorrhage and neonatal asphyxia between the two groups(P>0.05).The total comfort rate of delivery in the observation group was 91.49%,higher than that in the control group(74.47%)(P<0.05).Conclusion The left side inclined position for natural delivery can effectively accelerate the delivery process,reduce the rate of cesarean section,perineal tear,fetal distress,and obtain good delivery comfort,which is worthy of clinical promotion.
